DURING ANNUAL HEARING TEST WE FOUND A STANDARD THRESHOLD SHIFT IN EMPLOYEE'S HEARING TEST JUNE 29, 2009. WE HAD HIM RETESTED AND CONFIRMED THAT HE HAS HAD A STS IN HIS HEARING. HE HAS BEEN RETRAINED AS PER PART 62.
