EMPLOYEE WAS ATTEMPTING TO OPEN A WATER VALVE OFF A 2" HEADER. THE CENTER OF THE VALVE BLEW OUT. THE WATER UNDER PRESSURE STRUCK THE EMPLOYEE IN THE FACE. RUST PARTICLES IN THE WATER ENTERED T HE EYE. PARTICLES OF MATERIALS EMBEDDED IN EYE
EMPLOYEE WAS BREAKING BOULDERS WITH A PB-6 I.R. PAYEMENT BREAKER. EMPLOYEE LOST BALANCE, FELL DOWN WITH PAYEMENT BREAKER, PINCHED FINGER BETWEEN GRIZZLY RAIL AND PAYEMENT BREAKER, WHICH CAUSED DISLOCATION OF FINGER.
EMPLOYEE WAS TRAMMING MUCK FROM 7001 PILLAR STOPE.HE WAS DRIVING EMPTY TRAIN IN 62//HAULAGE DRIFT TO 7001.BY #6 VENT HOLE AT SEC. 33,DUST CONDITIONS EXISTED, AND APPARENTLY THAT IS WHERE EMPLO EMPLOYEE GOT FOREIGN OBJECTS IN EYE.EMPLOYEE RECEIVED RX.
APPARENTLY EMPLOYEE WAS GOING OUT WITH A LOADED TRAIN AND STOPPED AT 2500-3100 INTERSECTION TO MOVE A STULL THAT WAS LEANING INTO THE DRIFT. AFTER HE MOVED THE STULL HE CONTINUED GOING OUT WIT H HIS TRAIN, TRAVELED ABOUT 10\ WHEN HIS HEAD STRUCK A PIPE GOING ACROSS THE HAULAGE CAUSING HIM A BLOODY NOSE AND DIZZINESS
THE MUCK ON THIS ORE PASS HAD BUILT UP AT THE THROAT OF ORE PASS EMP WAS USING 6FT SCALING BAR TO SCALE IT DOWN HE WAS STANDING ON OUTSIDE OF ORE CAR WHEN SOME MUCK CAME DOWN AND HIT HIM KNOKK ING HIM INSIDE OF ORE CAR MORE MUCK CAME DOWN WHEN HE WAS INSIDE OF CAR HITTING HIM ON THE BACK
EMPLOYEE WAS WORKING ON ORE CAR AXLE, AXLE SWUNG OVER WHEN HE LIFTED UP THE AXLE, CATCHING EMPLOYEES FINGER BETWEEN CAR AND WHEEL

EE REPORTED TO ME THAT WHILE MOVING SOME 6X8X16, HE FELT NO PAIN UNTIL HE GOT HOME. NEXT MORNING HE REPORTED THAT HIS LOWER BACK WAS HURTING HIM FROM A PREVIOUS BACK INJURY THAT OCCURRED AT 3 304 DEVELOPMENT STOPE. FIRST AID WAS MADE ON THIS INJURY BY STEVE ERICKSON ON 8-23-83. EE DID NOT SEEK MEDICAL ATTENTION UNTIL 12-12-83.
EMPLOYEE WAS INSTALLING ROCK BOLTS IN THE BACK WHEN THE FOREIGN PARTICLE ENTERED HIS LEFT EYE. EMPLOYEE WAS USING A J-38 MACHINE AND JACK LEG TO DRILL HOLES AND TO INSTALL THE BOLTS.
EMPLOYEE WAS WALKING TO HIS WORK AREA THERE WAS A MOTOR OFF THE TRACK AT 1400 HDTHE ONLY WAY TO GET ACROSS WAS TO CLIMB OVER THE MOTOR WHEN EMPLOYEE CLIMBED OVER AND WAS GETTING OFF THE MTOR HER STEPPED ON A COUPLING AND LSOT HIS BALANCE AND FELL WHEN HE FELL DOWN HE LANDED ON HIS RIGHT HAND
OPERATING SLUSHER USING ALL 3 DRUMS.MIDDLE DRUM OPERATED WITH FOOT PEDAL.SLUSHING IN PILLAR STOPE.WHILE PUSHING DOWN ON FOOT PEDAL,FRONT BAND STOP BOLTS BROKE ENABLING BAND TO GO AROUND DRUM C AUSING PEDAL TO KICK UP SLAMMING EMPL FOOT AGAINST SLUSHER FRAME.
EMPLOYEE WAS TIGHTENING DRAIN ON DIESEL LOCOMOTVE, THE WRENCH HE WAS USING SLIPPED CAUSING EMPLOYEE TO HIT RIGHT ELBOW ON THE RIB OF THE MECHANIC SHOP. EMPLOYEE WAS FOUND TO HAVE CONTUSIONS TO RIGHT ELBOW.
EMPLOYEE WAS TRYING TO FIND TRACK TIE USING A SPLIT SET BOLT. CAUGHT FINGER IN GROOVE AND CUT FINGER ON BOTH SIDES.
EMPL THROUGH DRILLING ROUND.MOVING DRILL MACHINE AWAY FROM FACE,FELT PAIN ON LEFT SIDE OF GROIN.
EMPLOYEE WAS WALKING FROM 1ST SLUSHER POSITION TOWARD 2ND SLUSHER AND SLIPPED IN HOLE FOR STACKING MUCK, WHICH WAS EMPTY AT THE TIME, BRUISING HIS KNEE. EMPLOYEE DID NOTSEEK MEDICAL ATTENTION UNTIL 8/15/83.

WHILE EMPLOYEE WAS DESCENDING MANWAY LADDER AT 3306 EMPLOYEE SLIPPED AND FELL ABOUT 15FEET TO THE TRACK LEVEL EMPLOYEE WAS WEARING RUBBER GLOVES WHILE CLIMBING
THE EMPLOYEE WAS PROBING DOES NOT KNOW WHEN ANYTHING ENTERED HIS EYE WAS NOT NOTICES UNTIL AFTER WORKING HOURS AND EVEN THEN ONLY BY SIGHT AND NOT BY ANY IRRITATION TO THE EYE
EE WAS CLIMBING OVER A PARKED CAR IN 1000 HD & THE MOTORMAN MOVED THE TRAIN CAUSING ELLEN TO BE BUMPED AGAINST THE RIB.
