Employee had just gotten off work, he was driving out of the mine on the haul road and was trying to pass two other employees and got off the road and hit an old gate post.

Employee was climbing onto machine. He had 3 points of contact (left foot on track step, right foot on final drive, right hand on cab rail). When stepping up and shifting his weight, he lost his grip and balance, falling backwards. His hard hat came off, and he hit & cut his head on the bucket lip. He had a 2" cut closed by staples at a local hospital.

Employee was rolling a PC2000 boom pin around on a pallet when his hand slipped off the edge of the pin. When his hand went over the edge of the pin there was a thin piece of metal exposed that went through his glove and the middle finger on his right hand. The employee was transported to the er, the metal was pulled out of his finger, drs opened the wound for complete cleaning.

Employee was operating a PC400 trackhoe getting a rock truck un-stuck. After getting the truck un-stuck the employee was moving the trackhoe out of the way when he ran onto a out-crop of rock causing the trackhoe to turn over on the side. The employee was able to get out of the back window of the cab. He complained of a sore neck.

The employee states he was climbing down the ladder of a track hoe. He had parked his service truck near the bottom of the ladder and left the fuel door open under the ladder of the track hoe. When he descended the ladder he struck the top edge of the fuel door. He sustained a small laceration on his left buttocks.

The employee states he was starting to climb on to a D11 dozer to check the fuel, and his foot slipped causing him to fall on his left shoulder. The employee continued to work until taken off work on August 23, 2012 for physical therapy.

Employee was diagnosed with a knee strain and returned to work 01/11/12. Employee had surgery on right knee in 2008 from previous injury.

Employee was operating 100 ton rock truck. When dumping, the truck blew a hydraulic line. The fluid sprayed on the engine causing a fire. When he exited the cab he burned his arms and head. He also hit his head on the way out of the cab causing a cut that required stitches.
