The employee states he was climbing down the ladder of a track hoe. He had parked his service truck near the bottom of the ladder and left the fuel door open under the ladder of the track hoe. When he descended the ladder he struck the top edge of the fuel door. He sustained a small laceration on his left buttocks.

The employee states he was starting to climb on to a D11 dozer to check the fuel, and his foot slipped causing him to fall on his left shoulder. The employee continued to work until taken off work on August 23, 2012 for physical therapy.

Changing 4000-57, breaking down bead seat band and it fell out of tire

The injured employee, a supervisor, states he was lifting a bucket tooth for a front-end loader when felt pain in his left shoulder. He continued to work, but went off work for a surgical repair of his shoulder on 08/14/2012.

Employee was stepping out of his service truck and stepped down into a hole causing him to turn his left ankle.

Employee was shoveling coal on a screen. He felt a tingling in his right shoulder and down his right arm.

Employee pulled up to dump area driving Komatsu 785 truck #2527 turning 120' from dump area and backed up at a high rate of speed powering through berm. Material was 70 inches on either side of berm where truck breached the berm, traveling down 30' flipped on its top.

Employee was installing top shock on service truck # 6191 when socket fell from bolt hitting employee in mouth requiring 3 stitches inside of mouth and 4 stitches on the out side of his mouth. No lost time

Employee was dismounting from cat 775E rock truck, ee foot hung in step causing him to strain lower back and upper left leg

Employee stated he moved to the right side of the road to miss so bumps came back to the left side of the road loosing control laying rock truck # 2526 on it's side damaging hand rail, air breather and mirror.

Operator was dismounting from rock truck # 2518 and jumped from last two steps causing lower back lumbar strain.

Employee was operating Pc 1800 track hoe #6108 when a o-ring blew spraying oil over back of machine. Operator was going down steps using hand rails to see where leak was at and slipped and fell on hip, right shoulder and right leg.

While changing steel on #3154 driltech drill employee closed door on his left thumb causing an amputation below finger nail. Employee was taken to Brookwood Medical Center for treatment

Employee was bolting drisco pipe together using boom on service truck # 801 to stabilize pipe while bolting pipe together. Employee turned and walked into hook hitting his head on the hook requiring 5 sutures, no lost time.

Employee was returning from dump area in #2546 rock truck, towards pit hit a hole in the road causing pain in neck and lower back. Employee was transported to Brookwood ER in Birmingham. Employee is scheduled to see Dr. 3/19/12

Pulling hose with emulsion across shot pattern in mud and strained left knee.

A tech mechanic was removing a rock knocker on a rear tire of a rock truck, employee picked the rock knocker up and was putting it into the rear of his truck and was cut on the right forearm requiring 5 stitches at Brookwood ER in Birmingham. Employee did not miss any work.

Employee was climbing ladder of PC 2000 # 6112 hand slipped on wet hand rail from rain causing employee to fall to the left while right foot hung on step twisting right knee. Employee was diagnosed with a knee strain and returned to work 01/11/12. Employee had surgery on right knee in 2008 from previous injury.

Employee was stepping off a man trip (school bus) twisted his left ankle and fell to the ground. Employee was taken to Brookwood Medical Center and the diagnosis was left ankle sprained. Employee expected to be off until 1/12/12

Employee was greasing dozer blade and turnd and started walking and bumped left knee on blade causing pain and slight swelling

Employee was mounting #2527 rock truck placing right foot on step to pull himself up and felt pain in his calf of his right leg.Employee was taken to UAB West emergency room evaluated and released.Referred to Dr. and released to full duty on 10/05/11

Employee was pulling grease line from service truck #1301 to grease a rock truck. the employee turned/twisted and felt pain in lower back. Employee was taken to UAB emergence room for evaluation. Employee was released and told to see his family DR. and could report back to work on 9/26/11. Employee had previous lumbar strain. Reel on truck worked properly.

Oper exiting cab of 5042 D-11 dozer missed step fell to track then ground. Steps and handrails all in good condition. cracked ribs and damaged spleen.

Employee kneeled on one knee at bore-hole to retrieve blasting cap resulting in medial meniscus tear to left knee.

EE was walking across bench area stepped on a rock causing him to fall, twisting left ankle. Took EE to UAB Medical West emergency for treatment. Emergency room Dr. said EE had a Sprained Joint referred him to his family Dr. Family Dr. released EE to return to work next day

Employee was finished servicing PC2000, while walking back to his service truck he felt his left knee pop. The knee began hurting and swelling and he was taking to the ER.

Employee was getting on the Water truck when his foot slipped causing him to fall. While falling he attempted to catch himself and grabbed with his right arm. He felt pain in his right shoulder when he grabbed.

Employee was coming out of the cab on the PC1800 and missed a step coming down the stairs, he fell on the catwalk and tried to catch himself with his arm pulling a muscle in his back.

Employee was dismounting his dozer, his foot slipped on the lower push arm step, causing him to fall into the push arm with his torso.

Employee was using a pry bar when it slipped and hit his thumb.

Employee was getting out of the cab of his loader when he slipped causing him to fall to the ground striking his right shoulder and side.

Operator was getting loaded at the trackhoe when a rock was dropped into the bed of his truck causing the employee to be shaken and jarred around inside the truck.

Employee was operating a 100 ton rock truck. After dumping a load, the bed came down and jarred the truck, causing the employee to be shook and injuring his neck.

Employee was cranking a gas engine. He pulled on the starter cord which backlashed and caused him to strain his hand and wrist.

Employee was climbing down from his PC400 trackhoe when he slipped falling to the ground striking his face on a large rock.

Employee was getting on his dozer at the beginning of his shift, when he slipped and fell between the push arm and track. He injured his left shoulder and left hip.

Employee was walking down a drill bench when he felt a pulling in his back, employee stated he thought he made it worse when he climbed up the ladder into the drill.

Employee was getting off his D475 dozer slipped while still holding onto the handle and strained his left shoulder.

Employee was climbing off his loader and stepped down and twisted his left ankle.

Employee was servicing a trackhoe, when he climbed off the ladder stepping to the ground he twisted his left ankle.

Employee was helping with a water pump, he slipped on some rocks and fell hitting his right knee.

Employee was exiting his rock truck at the end of his shift. As he was climbing down the ladder his foot slipped causing him to fall approx. 10 feet to the ground. He landed on his left arm. Employee broke his left wrist.

Employee was operating 100 ton rock truck. When dumping, the truck blew a hydraulic line. The fluid sprayed on the engine causing a fire. When he exited the cab he burned his arms and head. He also hit his head on the way out of the cab causing a cut that required stitches.
