Tamrock Gravel Company · Fall to the walkway or working surface

EE tripped over his feet when walking to the loader. He fell on his right shoulder. He said it happened during the last week of January. He continued to work full time until 3/3/14 at which time we were done crushing so he went on job attached unemployment. He was to return 5/12/14 but felt his shoulder was only 80%. WC case was opened on 5/19/14 & he was terminated.

9/25 unclogging hopper on screen plant, removing (2) pieces of conglomerate. These pieces dropped down to pile of screened rock. Went to move conglomerate from pile; felt pull in lower back. 9/26 Wednesday morning experienced severe pain & reported to work & left to report to the office. Dr. appt. was made for that morning.

A WELD HAD BROKEN ON HOPPER GRIZZLEY ALLOWING A LARER ROCK THAN NORMAL TO GO INTO THE JAWS. CRUSHER OPERATOR SAW ROCK AND TRIED TO REMOVE IT FROM BELT CAUSING INJURY TO HIMSELF.
