EE received a laceration to his left index finger that required three stitches. He was preparing for a repair on a conveyor belt. While holding a new blade for a cutting knife in his left hand, he went to close a tool box and the blade slipped and caused a cut to his left index finger. After receiving the stitches at the local hospital the ee was returned to full duty.

While operating a loader, the employee was dumping material into a grizzly. The tilt control on the loader stuck allowing the bucket to roll forward hitting the bucket stops. This jarred the loader causing an injury to the employees lower back. Lower lumbar strain.
Employee was helping another employee remove a seat from a front end loader. The injured employee was in a ackward position during the removal process and strained his back. Strained Back
Employee was standing with one foot on a conveyor frame and the other foot on a cross brace of the conveyor tunnel wall. While he was striking the side of a material hopper with a hammer to clear some blockage, his foot slipped and he struck his back against the wall of the tunnel. Lower back contusion.
Employee was backing a backhoe down an incline. When he hit the brakes to stop, the backhoe slid sideways, struck a bank and turned over on its side. The employee struck his left foot against the rollover protection, causing a contusion.
Employee was walking down a slopped area, when he tripped and fell. He reached out with his right hand to break his fall, and felt a pain in his right hand. (Metacarpal fracture of right hand)

The employee was using a bar to pry up on a pulley shaft. They had a motor and gear box suspended from a come-a-long and the lifting eye broke. When the motor and gear box dropped, the employee's finger was caught between the pry bar and the conveyor rock box. (Fracture of the right ring finger)

Employee was removing a rock from a primary bypass chute, when the employee threw the rock over the side of the chute he caught his hand between the side of the chute and the rock. Left hand laceration
Employee was clearing a clogged material chute, he hit a large rock slab with a hammer and small peice of dirt flew up and went under his safety glasses and got into his eye. (superficial injury of cornea)
Employee was walking on walkway, stepped in dip on walkway and twisted knee. Sprained Knee (Right).
Employee was stepping up onto a ladder, and slipped, falling backwards. The fall resulted in a contusion to the back.
The injured employee and another employee had a disagrement which broke out into an arguement. The arguement escalated into a physical altercation, the second employee struck with his fist to the left cheek. Resulting in a laceration to the left cheek requiring nine stitches.
Employee was shoveling around conveyor, shovel hit conveyor belt and kicked it back striking the employee on the left forearm. Left forearm contusion.
Employee was walking to service vehicle, tripped over rock fell backwards causing left knee to twist. Employee didn't seek medical attention until 06/22/04
Employee was working under a screen, when he was walking out from under the screen he stood up before he cleared the frame holding the screen and struck his head on a frame cross member, resulting in a strained neck.

THE EE REACHED AROUND THE INSIDE OF THE DRIVE MOTO R TO FREE THE HOSE, WHEN HIS GLOVE GOT CAUGHT ONTHE DRIVE SHAFT NEXT TO THE MOTOR. EE JERKED HIS HAND AWAY FROM THE DRIVE SHAFT, AND CUT HIS HAND ON THE EDGE OF HEAD PULLEY GUARD.
REBUILDING 4000 AMP SERVICE PANEL, RECONNECTING EXISTING EQUIPMENT SERVICES, BACKFEED FROM DAMAGED CABLE ENERGIZED EQUIPMENT CAUSING PHASE TO PHASE SHORT. (MINOR FLASH BURNS)
AFTER INSTALLING A RUBBER GROMMET AROUND WIRING AT THE CLARIFIER, THE EE WAS APPLYING RTV SILICONE AROUND THE GROMMET TO HOLD IT IN PLACE. WHILE THE EE WAS SQUEEZING THE TRIGGER ON THE CAULKIN G GUN, THE EE FELT AND HEARD A POP IN HIS LEFT HAND RING FINGER. THE EE SUFFERED A RUPTURED TENDON ON HIS LEFT HAND RING FINGER.
THE EE WAS CHANGING A BEARING ON A HEAD PULLEY, THE SUPPORTS HOLDING THE HEAD PULLEY SHAFT SHIFTED AND CAUGHT THE EE'S LEFT HAND BETWEEN THE PULLEY SHAFT AND THE CONVEYOR FRAME. THE ACCIDENT R ESULTED IN AN OPEN WOUND AND CONTUSION OF FINGER ON LEFT HAND, WITHOUT MENTION OF COMPLICATION.
WORKER WAS DELIVERING FUEL AT ALL-LIGHT AGGREGATE. HE STATED, HE TRIPPED OVER DEBRIS WHILE HE WAS HOOKING UP PUMP HOSE TO DELIVER FUEL. HE STATED THAT THE AREA HAD NO LIGHTING.
EMPLOYEE WAS WALKING DOWN A SLOPED AREA FROM THE UPPER PLANT TO THE LOWER PLANT TO PICK UP A HAUL TRUCK, HE SLIPPED AND FELL, LANDING ON HIS LEFT HIP. THE EMPLOYEE WAS DIAGNOSED WITH A DEEP TISSUE BRUISE TO HIS LEFT UPPER THIGH AND HIP. THE EMPLOYEE LEFT THE LIGHTED WORK AREA WHERE VISIBILITY WAS POOR AND THE GROUND CONDITIONS WERE SLIPPERY DUE TO RAIN.

WELDING ON DUMP HOPPER. TEMPORARY SCAFFOLDING BROKE LOOSE AND FELL TO GROUND. (5-6' HIGH).
SLIPPED ON FLOOR DRY. FALLING FORWARD HAND WENT THROUGH WINDOW. WITNESS SAID EE WAS KNOCKING ON WINDOW (WINDOW WAS LATCHED CLOSED).

WORKING ON 992C LOADER. CHECKING ACCESS TO RUN WIRE FROM CAB TO ENGINE. COMMING FROM UNDER CAB HE GRABBED HAND RAIN AND CAB, THEN RAISED UP AND SLIPPED, CAUSING HIM TO HIT MIRROR ARM AND THEN FALLING FORWARD TO THE DECK.
MAN WAS GREASING CONVEYOR WHEN IT STARTED TO RAIN. HE SLIPPED ON WET BELT AND FELL TO GROUND ABOUT 6 FEET. HE BRUISED HIS THIGH.
EE WAS CHECKING OIL AND TURNED ON MASTER SWITCH ON WATER TRUCK WHEN HE LOST HIS BALANCE. HE STRUCK THE BUMPER ON WATER TRUCK AS HE FELL TO THE GROUND AND HE BROKE SEVERAL RIBS AND CRACKED A VE RTEBRAE. HE FELL APPROX. 4'.

EE WAS INSPECTING S.B.M (CRUSHER)ROT0R. HE HAD HIS HAND PUSHING THE ON A BLOW BAR TO ROLL THE ROTOR. WHENTHE ROTOR ROLLED BACK TO WHERE THE BLOW BAR WAS ONTOP, THE BLOW BAR FELL INO ITS SEAT IN THE ROTOR, WHEN IT MOVED TO ITS SEAT, HIS FINGER WAS CRUSHED AND HE IMMEDIATELY UPLLED IT OUT.
EMPLOYEE WAS PUSHING MATERIAL OFF TOP OF JAW STOCKPILE WITH D8K DOZER. A LARGE ROCK ROLLED DOWN INTO CUT WHILE HE WAS BACKING UP. THE DOZER TRACK BOUNCED OVER ROCK. BACK WAS INJURED.
