Miles Sand & Gravel · Struck by flying object

Miner removed bolts thought to secure "cap" protecting an oil sensor threaded and bolted into a pressurized tank. The cap was then dislodged from silicone seal causing the release of pressure inside the tank and cap to strike the miner in the jaw. Other sensors had been accessed prior to this all located under bolted on protective covers not under pressure.

Miles Sand & Gravel · Fall to the walkway or working surface

Freezing conditions over the weekend of December 17 & 18 created frozen and slippery ground. Shortly after the beginning of the shift, the miner was walking across the gravel mine, and slipped and fell on the frozen ground.

Concrete Nor'West · Caught in, under or between a moving and a stationary object

Injured on 3/30/12 loading material into loader bucket and foot was smashed under bucket. The emp. was evaluated @ ER and no med. treatment was administered and the injury was limited to bruising only. On 5/17/12 the emp. returned to the Dr. and a walking cast was applied. We feel this now meets the definition of med. treatment, therefore we are submitting this report as required.

Concrete Nor'West · Over-exertion in lifting objects

Accident was NOT reported to Concrete Nor'West until 3/25/09. On approx. 2/25/09 EE was carrying bags of cement up a hill. On 3/20/09 he was having muscle spasms and saw a chiropractor. He saw the Chiropractor again on 3/24/09 and the doctor indicated he has compression fracture. EE informed us on 3/25/09 that he injured himself in February 2009.

Saw doctor on 6/12/06 with pain to hip area. Bacterial infection set in at rt knee, rt flank & rt hip. On 6/23/06 realized he had scraped his rt knee on a concrete block, which was the source of the bacterial infection. Off work for approx. 3-4 weeks.

Employee was replacing a safety guard in the tunnel. Employee was lifting the safety guard and turning. Employee felt back go out.
