A repair supervisor was injured after obtaining the location of a hydraulic hose on a haul truck. The location was needed to facilitate the repair of a similar truck. After obtaining the information, the employee stumbled and fell while returning to the step ladder. The fall resulted in a fractured collar bone.

EE WAS REPLACING THE BACK, RIGHT FEEDER BOLT ON THE SYNTRON FEEDER. A COME-ALONG WAS BEING USED: ONE HOOK ATTACHED TO A CLEVIS ON THE FEEDER, THE OTHER HOOK ATTACHED TO THE FEEDER ROOF HANGER BOLT. EE WAS HAND-TIGHTENING THE FEEDER BOLT WHILE ANOTHER EE WAS LIFTING WITH THE COME-ALONG. THE FEEDER ROOF HANGER BOLT BROKE CAUSING THE SPRING CLUSTER TO FALL. HIS HAND WAS CAUGHT IN IT.
EE WAS IN THE PROCESS OF REMOVING THE DISTRIBUTOR PLATE FROM THE HEAD OF A 1560 OMNICONE CRUSHER WHEN HIS RT FOOT SLIPPED DOWN INTO THE CAVITY CAUSING HIM TO STRAIN LIGAMENTS IN HIS KNEE.
