Employee was getting a 2100 screen on the lowebed trailer ready to mobilize. Employee was placing blocks by the goose neck as they were standing on the tires of the truck. Employee's left foot slipped, employee fell landing on their right side on fender of the truck.

Joy Engineering · Caught in, under or between running or meshing objects

Plant laborer did not have supervisor follow Lock Out/ Tag -Out procedures prior to attempting to free a rock jammed conveyor. Upon removal of the debris at the tail pulley the conveyor began to run, at which time the laborers arm was pulled into the tail pulley resulting in a broken right arm above the elbow.
