EMPLOYEE WAS BOLTING TOP AT THE TIME OF THE ACCIDENT. HE WAS SWINGING BOOM TO PUT UP MIDDLE BOLT WHEN HE LET THE BOOM DOWN ON HIS LEFT KNEE.
