Miner scheduled eye doctor appointment after complaining of eye irritation. Finished shift and went to doctors following day. Piece of steel was found in eye.

There were scattered showers prior to the accident The Terex began to slide on the final slope entering the crusher floor EE steered to the right. While making the turn the articulated dump box turned over towards the left by the momentum the cab section remained in the upright position.

Miner was attempting to remove a tooth from excavator bucket with hammer when a small shard of metal came off of bucket and struck miner in the thigh.

EE was using a piece of metal/pipe to clear an aggregate blockage at the Crushing Plant Chute. When the blockage began to clear, pieces of loose aggregate fell on ee right hand. The falling aggregates caused two small cuts, a swollen middle finger and some numbness in the pinky finger.

Employee was pulling a welding cart w/Acetylene & Oxygen behind him. The cart hit a ditch, he lost control and was struck behind his knees with the handle of the cart. Received a bruise and soreness in back of leg.

EMPLOYEE WAS OPERATING A BULLDOZER PUSHING ROCKS THAT HAD BEEN BLASTED FROM A HILL WHEN A ROCK CAME LOOSE FROM THE FACE OF THE HILL AND FELL EMPLOYEE'S LEFT FOREARM.
