E.E. WAS OPERATING PETTIBONE, ATTEMPTING TO REMOVE 6"-12\ I BEAM THAT WAS STANDING ON TO 12\ POLES OF 4" PIPE. FORKS RAISED THE I BEAM, THE WELD BROKE CAUSING THE BEAM TO SLIDE OFF THE FORKS A ND STRUCK ANOTHER EMPLOYEE WHO WAS STANDING NEARBY.
