EE was chipping away at a rock stuck in the 4-8 chute when the blockage cleared the material finished falling to the belt. Bar was pushed into EE's shoulder.

Employee was throwing rocks off conveyor and hit hand on guard, which cut right hand middle finger. Employee wasn't wearing gloves when incident occurred. Then after work had gone to get 9 stitches.

On 1/8/2020 I was notified that an employee was on the way to the hospital for an infection in their knee. After talking to the employee they said they were using the cutting torch when a piece of slag burned their knee. This occurred on 12-10-2019 to the best of their recollection.

G & S Gravel Inc · Fall from scaffolds, walkways, platforms

Contractor fell from cone crusher platform. He was not wearing his required safety harness. He was treated by paramedics & transported to John C Lincoln Hospital & released later that evening. He suffered a back injury. At the time of the accident I was in California and not present on the mine.

WHILE USING A ROCK DRILLER AND DRILLING INTO A ROCK FACE THE VIBRATION CAUSED A 10-14 INCH ROCK TO FALL OFF AN 8 FT CLIFF ABOVEE AND LAND ON HIS FOOT.
