USING ALLEN WRE3CH & HAMMER TO TIGHTEN BOLT TO PROPER TORQUE-WRENCH CAME OUT OF BOLT WHEN HE STRUCK IT SMASHING HIS HAND AND FINGERS AGAINST SPIDER GEAR.
EMPLOYEE WAS UN5OADING A SEMI LOAD OF 55 GALLON DRUMS WHICH WERE DOUBLE STACKED. WHILE MANNIVERING THE DRUMS HE SMASHED THE FINGER OF HIS RIGHT HAND.
