EE WAS LOADING A FILE CABINET ON HIS TRUCK. HE STUMBLED AND FELT SOMETHING PULL IN HIS BACK AND STOMACH.

EMPLOYEE WAS PULLING ON A RAMP, UNLOADING FROM A PICK-UP TRUCK TO A FUEL TANK. HE FELT A STINGING IN HIS LEFT SIDE. HE FILLED OUT A COMPANY ACCIDENT FORM AND CONTINUED TO WORK. HE WENT TO HIS FAMILY PHYSICIAN 12/12/00. WAS INFORMED HE NEEDED SURGERY FOR A HERNIA. THE SURGERY WAS SCHEDULED ON 12/22/00. NO LOST TIME UNTIL 12/26/00.
