Emp wanted wait see if would be ok but swelling worsened, Supervisor took emp to clinic 03/08/19. Emp prescribed Tylenol, Diclofenac sodium, air boot for up to 6 hrs per day and physical therapy. PT noted boot was not effective after day 3 recommending boot is not required.

This injury is stemming from a first aid injury that occurred 9/10/14 and was reported at that time but no further medical treatment was sought. This was again reported 02/10/17 with the employee stating it was due the the injury in 2014. Original first aid was due to back strain as a roving mechanic at that time. *EE was shoveling on 2/10/17 per call.
