Emp exited water truck end of shift stepping onto uneven pavement rolling ankle. Injury treated with RICE. Emp wanted wait see if would be ok but swelling worsened, Supervisor took emp to clinic 03/08/19. Emp prescribed Tylenol, Diclofenac sodium, air boot for up to 6 hrs per day and physical therapy. PT noted boot was not effective after day 3 recommending boot is not required.

This injury is stemming from a first aid injury that occurred 9/10/14 and was reported at that time but no further medical treatment was sought. This was again reported 02/10/17 with the employee stating it was due the the injury in 2014. Original first aid was due to back strain as a roving mechanic at that time. *EE was shoveling on 2/10/17 per call.

On 05/20/2011, the employee was taken to the hospital and was admitted for more than 24 hours. The employee stated that while descending the ladder of a haul truck, he fell and bumped his head.
