WHILE BACKING THE HAUL TRUCK UP THE RAMP TO DUMP PRODUCT OVER BERM FOR THE CRUSHER FEED, THE RIGHT REAR OF THE HAUL TRUCK DROPPED & THE BANK GAVE WAY; CAUSING THE HAUL TRUCK TO TIP OVER. THE TRUCK APPEARED TO HAVE STRUCK THE BERM AT AN ANGLE.
WHILE WALKING FAST AROUND THE END OF PLANT, EE'S LEFT ANKLE GAVE WAY. EE REPORTED THAT HE HAS HAD PROBLEMS WITH THIS ANKLE BEFORE, ALL THE WAY BACK TO HIGH SCHOOL.
