WHILE BACKING THE HAUL TRUCK UP THE RAMP TO DUMP PRODUCT OVER BERM FOR THE CRUSHER FEED, THE RIGHT REAR OF THE HAUL TRUCK DROPPED & THE BANK GAVE WAY; CAUSING THE HAUL TRUCK TO TIP OVER. THE TRUCK APPEARED TO HAVE STRUCK THE BERM AT AN ANGLE.
