Employee was tramming the forklift toward the refuse to replace a tote at the portal. The forklift got too close to the edge of the road and the bank gave way.

He was walking down steps from control room, halfway down his left foot slipped. He went down and hit his left hip and his leg went under him. He was holding on to the hand rail with his right hand and slipped down about 3 steps.

EE was performing maintenance inside tipple, he was using a pry bar which slipped and pinched his finger.
He was making repairs to an excavator and was stepping onto the upper portion of an excavator. He lost his footing and fell off of the machine.
He was dumping a load of reject half way up the hoist pin broke bed came down with our the help of hoist.

Injured was helping install a French drain through old dam in King Branch; upon exiting the area he stepped on a rock. The rock twisted from beneath his foot resulting in a fall injuring his left leg. First aid was administered by two M.E.T.s.

EE was bent over cutting a piece of metal when he strained his lower back. EE missed 5 days and was on restricted duty until 11-24-04.
EMPLOYEE WAS WORKING ON THE WATER TRUCK WHEN HE GOT HIS FINGER MASHED WITH A HAMMER. HE CONTINUED TO WORK UNTIL 7-26-04 WHEN HE WAS PLACED UNABLE TO RETURN TO WORK. HIS FINGER IS BROKEN AND HAD TO BE RESET.

A METAL GUARD WAS BEING REMOVED SO ENTRANCE COULD BE ACCESSED TO THE COAL CRUSHER. THE GUARD SLIPPED OFF THE HOIST HOOK AND STRUCK EE, RESULTING IN LACERATIONS TO HIS STOMACH AND FOREARM. THES E CUTS REQUIRED STITCHES.

EMPLOYEE WAS WELDING STEEL W/AEC WELDER.FOREIGN BODY LODGED IN LEFT EYE.EMPLOYEE THOUGHT HE WASHED IT OUT.EYE BEGAN BOTHERING HIM AFTER WORK.DISCOVERED F/B IMBEDDED IN LEFT CORNEA.
