THE INJURED BROKE HIS ANKLE WHEN HE STEPPED ON THE FIRST RUNG OF THE JAW CRUSHER LADDER. THE GROUND WAS LEVEL AND DRY, THE FIRST STEP WAS APPROX 14" OFF THE GROUND. HE STEPPED WITH BOTH HANDS ON THE RAIL AND BEGAN TO CLIMB. HIS LEFT ANKLE TURNED AS HE STEPPED TO NEXT RUNG. HE FELT A SHARP PAIN IN L ANKLE & LOST GRIP, FALLING BACKWARD TO GROUND. NO INJURY WAS SUSTAINED FROM FALL.

EMPLOYEE WAS ATTEMPTING TO REMOVE A ROCK WHICH WAS BRIDGED AT THE OPENING OF THE JAW CRUSHER. HIS HAND BECAME PINCHED BETWEEN THE OPENING CROSS BRACE AND THE ANCHORED CHAIN OF THE ROCK GUARD. LEFT HAND WAS BRUISED AND CUT BETWEEN THE 4TH AND 5TH FINGERS REQUIRING 5 STITCHES.

HIS LEFT SIDE AND SHOULDER WERE BADLY BRUISED BUT HE HAD NOT BROKEN BONES. DR TOLD HIM TO STAY HOME FOR 4-5 D
