EMP WAS REMOVING DRILL ROD FROM RAISE BORE ROD HOLDER HE HAD INSTALLED THE HOIST CHAIN AROUND THE ROD AND WAS WAITING TO HAVE THE ROD RELEASED FROM THE CLAMP OF ROD HOLDER AS THE CLAMP WAS RE RELEASED THE ROD SLIPPED DOWN OUT OF THE CHAIN THROUGH THE ROD HOLDER AND STRUCK HIM ON RT FOOT
