Double D Mining LLC · Struck against a moving object

While preparing the pad to load hauling trucks; the side wall gave out causing the excavator to slide down approximately 12 to 15 feet; once the excavator got to the bottom, it tipped on it's side.

Clear night, no wind. Employee was welding when a hot piece of slag fell and burned threw the tongue of his boot burning into his foot.
