Employee was removing a roller out of an unused scavenger magnet. When the retaining bolt was removed the employee thought it was still supported. As the primary bolt was removed the roller fell and landed on the employees finger causing a laceration.

The ground had become deeply rutted by the repeated haulage of material in that area. A slight layer of ground water had also covered the area making the ruts difficult to see.

Crew was instructed to remove 2 end bolts on a newly installed launder to move a piece of 2 inch angle iron. As the employee removed the bolts the launder swung down to the floor striking the coworker. The crew did not recognize that none of the bolts in the middle section of the launder had been installed. Only two bolts at each end of the launder had been installed.
Employee was taking down the light tower on a light plant. The employee was rolling the winch down and the handle slipped out of employee's hand and came around and hit right hand.

Employee tripped and fell striking right shin.
Employee was installing a 7-8 foot long water pipe and was trying to start threads into a 90 degree elbow. The opposite end was resting on a metal box. As ee turned the pipe trying to start the threads, the pipe rolled off the top of the box causing the pipe to shift and mash the employee's finger between the pipe and corner of the equipment.
