TSJ Construction · Accident type, without injuries

Operator was pushing downhill on variable ground when after finishing a downhill push, backed the dozer uphill at an angle not perpendicular. Operator was alone and states that upon reaching close to the top, the dozer hit a hard crust on one track and soft on the other causing it to slide and tip on its side. Operator wore seat belt, uninjured.

TSJ Construction · Struck against stationary object

Miner was working on crusher changing screens when the screen slid and caught EE's hand. In an effort to avoid significant injury, EE retracted EE's hand and in doing so caught EE's (R) middle finger on a sharp edge that cut through the glove and lacerated finger. Medical treatment was administered at clinic and required a few stitches. Returned to work following day.

Pre-existing condition stemming from heart problems and malfunctioning defibrillator. While operating a water truck, miner experienced chest pain and dizziness and passed out after exiting the truck and asking for help.
