Olmsted Aggregate, Inc. · Flash burns (welding)

Employee was working in the shop and in the proximity of another employee that was welding. Injured employee states EE sustained flash burns from the welding operations. Employee was seen at a medical facility and given eye drops to use for the next few days.

Operating a front end loader, got out to help truck driver with a piece of rock stuck in a tailgate. When removing rock finger was caught and the end was cut off.

SCALE ATTENDANT WAS SWEEPING THE OFFICE FLOOR. FOOT CAUGHT THE BASE/WHEEL OF A DESK CHAIR RESULTING IN A FALL AND SUBSEQUENT INJURY TO THE LEFT UPPER LEG OR HIP.
