THE EMPLOYEE WAS BACKING UP A HAUL TRUCK TO GET LOADED, WHEN THE DOOR THAT WAS OPEN FOR VENTILATION CLOSED PINCHING FINGERS OF LEFT HAND BETWEEN DOOR AND FRAME.
EMPLOYEE WAS WALKING TO A-25 TRUCK AND HE TRIPPED OVER EXPOSED WIRE ON THE GROUND CAUSING THE EMPLOYEE TO FALL. EMPLOYEE SUFFERED ACUTE LOWER LEFT BACK SPRAIN.
EMPLOYEE STEPPED INTO A HOLE AND TWISTED THE RIGHT ANKLE. X-RAYS REVEALED A CHIP FRACTURE. EMPLOYEE RETURNED TO WORK THE FOLLOWING DAY. THE EMPLOYEE RETURNED FOR A CHECKUP AND THE DR. DECIDED TO PERFORM ARTHROSCOPY W/ DEBRIDEMENT AND REMOVE LOOSE BODY PARTICLE ON 9-10-97. THE EMPLOYEE WAS OFF WORK UNTIL 9-30-97. INJURY STARTED AS A FIRST AID INJURY AND PROGRESSED TO THE OPERATION S
