WHILE TALKING TO ANOTHER EMPLOYEE HE PICKED UP WHAT HE THOUGHT WAS A PEN OR HOLE PUNCH WHICH TURNED OUT TO BE A GUN AND IT WENT OFF IN HIS HAND.
EMPLOYEE WAS INSTALLING MAT ON SCREEN MAKING HOLES IN MAT WITH RAZOR KNIFE. KNIFE SLIPPED AND CUT LEFT FORE ARM.
EMPL ADJUSTING A 2" NUT ON THE END OF THE TOGGLE SPRING AND THE WRENCH SLIPPED OFF THE NUT AND FINGER WAS SMASHED AGAINST THE JAW HOUSING AND THE WRENCH.
