ORE BEING FED TO PELLETIZER WAS MUDDY FOLLOWING HARD RAIN ON PRECEDING EVENING. EMPLOYEE WAS ATTEMPTING TO REMOVE MUD BUILD-UP FROM TAIL PULLEY USING A SHORT METAL BAR. BAR ANBD HAND BECAME CU AUGHT BETWEEN BELT AND PULLEY. (CONVEYOR WAS RUNNING). HAND AND FOREARM WERE PULLED BETWEEN BELT AND PULLEY.
