Employee was lifting a conveyor bent section off the work table; one end was resting on the floor. The employee lifted the other end off the table and was lowering it to the floor when he felt a burning sensation in his lower back. The bent is 12" x 4' and weighs approximately 150 lbs.

EMPLOYEE HAD CUT A 45 DEGREE CUT ON AN I-BEAM. HE WAS PUSHING THE CUT I-BEAM FROM ONE TRANSFER ROLLER CONVEYOR TO ANOTHER. THE END OF THE I-BEAM STRUCK THE FIRST ROLLER ON THE SECOND CONVEYOR AND STOPPED. HIS HAND WAS CUT ON THE END OF THE I-BEAM.

EMPLOYEE WAS ASSEMBLING A SCREEN CHUTE INSIDE A SUPPORT FRAME. A COME-A-LONG WAS ATTACHED TO THE CHUTE AND TO A LIFTING EYE THAT WAS TACK WELDEDONTO THE FRAME. WHILE ATTEMPTING TO PULL THE 2 PIECES TOGETHER, THE WELD BROKE AND THE EMPLOYEEWAS STRUCK IN THE MOUTH.

THE EE WAS CUTTING THE END OF AN I-BEAM THAT WAS RESTING ON TWO SAW HORSES. WHE THE END WAS CUT OFF, THE BEAM BECAME UNBLANCED. AS THE BEAM STARTED TO FALL, THE EMPLOYEE TWISTED AND REACHED OU T WHEN HE FELT A PAIN IN HIS BACK.

EE WAS WELDING A CRACK IN THE SOLID SHOVEL BOOM. HE WAS UNAWARE THAT GREASE HAD COME FREE AND WAS TRAPPED WITHIN THE BOOM. THE WELDING ACTIVITY HEATED THE GREASE TO ITS FLASHPOINT. THE GREASE EXPLODED AND WAS FORCEBLY EXPELLED FROM THE BOOMONTO THE EE BURNING HIS LEFT WRIST.

TWO EE LIFTED A 20' LONG PIECE OF FLATBAR OFF OF A PAIR OF SAW HORSES. ONE EE WAS POSITIONED AT EACH END. AS THEY LIFTED IT OFF, THE PIECE BOWED IN THE MIDDLE SHIFTING THE LOAD. THE INSURING J ERK CAUSED THE EE TO STRAIN HIS BACK.

THE EE WAS USING THE FORKLIFT TO MOVE A HEAVY PIECE OF STEEL TO A LOCATION IN FRONT OF THE SHOP. WHILE TRAVELING ACROSS THE DRIVEWAY, HE HIT A HOLE CAUSING THE FORKLIFT TO BOUNCE. THIS IMPACT CAUSED HIM TO HURT HIS BACK.

EE WAS LIFTING BUCKET HE WAS WORKING ON, HE HOOK SET OF SPREADER TO BACK OF BUCKET. HE PUT TENSION ON HOOK ONE CAME LOOSE. HE STEP IN FRONT OF HOOK TO RELOCATED HOOK, HIS INTENSION WAS TO LWER HOIST AND REHOOK WHEN HE HIT THE UP BUTTON INSTEAD OF DOWN PUTTING TENSION ON THE OTHER HOOK, WHICH THEN POP OFF STRIKING EE IN MOUTH, BREAKING 2 OR 3 TEETH, CUT FOREHEAD.

EE WAS AIR ARCING ON A LOADER BUCKET WITH HIS FOOT RESTING ON A CHAIR, WHEN A PIECE OF CARBON ROD BROKE OFF [APPROX 1 1/2" LONG] CATCHING IN PANTS LEG. WHEN EE TRIED TO KNOCK OF ROD IT FELL DO WN INTO HIS BOOT, BURNING THE BACK & FRONT OF LOWER CALF & THEN DOWN ONTO FOOT.
