The associate was climbing up the ladder of a Komatsu WA 500 and slipped of the ladder thus falling 4 - 5 feet. EE landed on EE's right elbow. The elbow suffered abrasions and a contusion.
Employee was walking and checking drill holes and accidentally stepped into a drill hole. This caused their foot to collapse despite wearing steel toed boots. This resulted in a fracture in their foot.
The employee had a knife in one hand with a piece of conveyor belt in the other . While making a vertical cut, the razor blade detached from the knife handle and cut EE's hand.
Employee was kicking rocks off the undercarriage of a conveyor belt when EE felt pain on the back of EE's lower left leg.

Employee was helping Maintenance Dept. perform electrical grounding checks. EE was descending a permanently affixed ladder. EE pinched left index and middle fingers in the ladder. EE had to get a total of 10 stitches in the left index and middle fingers and fractured the very end of the middle finger.

There was apparently a flammable gas in a plastic pipe that ignited while the employee was close to it. It caused debris to fly onto the left side of EE's face and left eye.
While operating an excavator, A rock fell off the truck being loaded and struck the employee's lower right arm. This resulted in the employee having 2 stitches.

The associate was taking a hydraulic cylinder off of a crane on a lift boom and slipped off the front of crane. EE struck right wrist and caused a laceration on their right eye brow.
The Employee was changing a battery in a Caterpillar 992. EE had an associate holding the battery cover up and also had a prop under the battery cover. The associate let go of the cover to grab a wrench and the wind blew causing the battery cover to fall and strike right hand.

An employee was opening the inspection door to a rock crusher when the door slipped out of his hand and hit his left forearm. This resulted in a contusion of the left forearm. He will be limited to only using his right arm for what appears to be a few days.

Employee was adjusting the outrigger on his mechanic's truck. Held on to outrigger unit by placing his finger in the lowest adjustment setting hole. His hand slipped and the outrigger foot fell trapping his finger between the outrigger and the outrigger housing.

Employee was attaching grease lines on haul truck, Dirt fell from the grease line and struck his eye. Employee was wearing safety glasses.
Employee states the he was pulling on hose to clean up Steadman crusher and injured his left shoulder. This was reported to the supervisor around 9:00am on 9/25/12, company was notified that employee had MRI which indicted a torn rotator cuff.
Employee was repairing an electric motor and hoisting it when his right pinky finger got caught between a cable and the motor. He had to get six stitches.

Employee states he fell while getting off of a loader. He alleges he mis-stepped near the last step of the loader & fell to the ground. He tried to catch himself with his arm & fell on it as well. He originally thought the fall was not serious. He had continued pain and saw a doctor that put him on modified dutyuntil Oct 5th. The Dr. susequently took EE off of work.

Employee was driving a maintenance truck down the quarry road when he lost control of the truck & it flipped. He was trapped in the vehicle until emergency response personnel retrieved him. He received stitches in his ear & was bruised. He was seen at the ER & discharged on same day. He was instructed to not return to work until results of next checkup on 5-13-09.
Employee states he was trying to lift a motor from a shelf and that he hurt his lower back in doing so. Employee originally continued working but has since experienced continued pain. Employee is presently scheduled to have back surgery in May and is not working due to an enlarged disc.

While repairing an engine, employee was trying to remove a part with a pry bar when it slipped causing him to cut the top of his left thumb.
Employee originally stepped awkwardly in a dip in the ground on 1/8/08 hurting his right knee. Employee continued to work. He then states that he re-injured the same knee on 2/16/09. He continued to work. His knee has evidently worsened and he had knee surgery on 3/27/09. Employee has since returned to work, seated duty only, on 4/15/09.

Employee was operating a haul truck. He alleges that the seat collapsed. He alleges that his back was injured by the alleged seat collapse. He took a week's vacation the next week and resigned from the Compay on July 6, 2006.
EE was helping change a rock crusher hammer with another EE. During the process, the EE got his finger caught under the hammer causing it to be pinched and therefore cut causing 6 stiches to be received.

Employee crawled through an opening to clean the bottom deck of a screen. While attempting to crawl out of the screen, he struck his right knee on the bottom of the screen housing

WORKED THRU FRI, WAS SCHEDULED TO WORK SAT & SUN BUT DIDN'T. WENT TO DR SUN. 5/12, WHO SAID IT WAS A PULLED MUSCLE & PRESCRIBED MEDS.
EE CLAIMS HE HURT HIS BACK WHILE WORKING. EE STATED THAT ON THE NIGHT OF 4/16/02 HIS BACK STARTED GETTING SORE. THEN HE STATED ON 4/17/02 HIS BACK WAS HURTING SO BAD THAT HE SAID HE NEEDED TO GO TO THE HOSPITAL. HE WENT TO DOCTOR 4/18/02. HE TOLD THE DOCTOR THAT HE HURT HIS BACK KICKING A PIPE.

EE WAS SITTING IN HAUL TRUCK. A LOADER OPERATOR WAS LOADING THE ROCK TRUCK WITH ROCK WHEN THE LOADER ACCIDENTALLY BUMPER THE ROCK TRUCK BED. AS A RESULT OF THIS CONTACT, THE EE REPORTED A STRA INED RIGHT SHOULDER.

THE EMPLOYE EWA SMOUNTING A V-BELT SHEAVE WITH THE HELP OF ANOTHER EMPLOYEE AND FELT A BURNING SENSATION IN HIS LOWER BACK. NO PAIN WAS INVOLVED THEREFORE THE EMPLOYEE CONTINUED TO WORK. LAT E IN THE AFTERNOON, THE EMPLOYEE'S BACK BEGAN TO HURT. NO CONDITIONS OUT OF THE ORDINARY WERE NOTICED.
THE EMPLOYEE WAS USING A WELDING TORCH ON 1-30-93 AND NOTICED A BURNING SENSATION IN HIS EYES ON THE EVENING OF 1-30-93. HE EXPERIENCED SOME DISCHARGE FROM HIS EYES ON THE EVENING OF 1-30-93. UPON TREATMENT HE WAS FOUND TO HAVE RUST PARTICLES IN HIS EYES.
