Grinding a piece of metal 3/8" diameter off return roller guard. Metal shavings got in his left eye. He did have on safety glasses at the time. Metal shavings probably got in eyebrow and when removing safety glasses, got in the eye.

Cleaning off grizzly bars - swinging sledge hammer, pulled muscle in back or pinched nerve. Said he wanted to go to doctor to get checked out 8/23/10 12:00pm.

Atv's or Quads-tresspassing onto the property (nightwatchman confronted one or stopped one, nightwatchman had a shotgun, gun accidently went off killing one of the riders. No witnesses on company behalf. Non mine related.

Installing new conveyor belt, cut belt to size in order to fasten together. When cutting belt he was kneeling on the belt pulling knife towards himself, knife slipped out and he stabbed it in his right leg. Cut about 1" to 1 1/2" long and 1/2" wide.
