THE LOADER OPERATOR WAS DESCENDING FORM MACHINE USING THE ACCESS LADDER WHEN HIS FOOT SLIPPED AS HE WAS SHIFTING HIS HAND HOLD. TO AVOID FALLINGHE GRABBED QUICKLY AT THE HAND RAIL ALONG THE LA DDER WERE IN PLACE AND IN GOOD SHAPE. OPERATOR WAS EXCERCISING NORMAL CAUTION IN DESCENDING. THERE WAS NO FALL.

THE EMPLOYEE WAS HELPING TO REPAIR A DOZER TRACK WHEN THE PRY BAR HE WAS USING SLIPPED CAUSING HIM TO LOSE HIS FOOTING. HIS LEFT LEG WENT OUT TO THE SIDE STRAINING HIS LEFT KNEE.
