EE had gone to the restroom, walked out and put hard hat on and went to put safety glasses on and the arm of the glasses went in eye.

Employee was replacing a screen panel. Employee stated the they weren't feeling well and was taken to an air conditioned MCC room to cool off. Employee was not recovering as expected and EMS was called. Employee was taken to the ER and treated for heat exhaustion.

Employee was using "Dog and Wedge" to better position a clad cheek plate, striking pieces with hammer. Fragment from steel hammer broke off hammer enter eye from below safety glasses. Fragmentation caused from steel to steel strike.

Employee was cutting a small piece of belting and pulled the knife in to employee's right leg above the knee. Making a 1 inch cut

Employee was servicing the loader 1078076 when EE felt a sting in EE's left shoulder blade area, brushed a spider off of EE's back.

While installing the mounting flange to the wash plant waste pump, the flange fell off the housing seat approximately 2" smashing his left thumb between the flange and shaft of the pump. His thumb required some stitches but no broken bones.

While closing a sliding window in the crusher shack, the window came off track. EE pushed on the window to put it back on track and the window glass broke cutting his right hand.

While removing cutting edge bolts from a cat 988H loader, EE used his hand to push up a volt to lock it into place. At the same time he hit the trigger of the impact wrench with his other hand causing the bolt to spin in his hand. The worn bolt head cut his thumb through his leather glove. He was wearing all requited PPE and the loader was locked out and tagged.

Cutting belt holes in belting, knife slipped and cut left thigh.

Employee was stepping down from truck onto the ground. He lost footing and landed awkward onto the ground injuring his ankle.

WHILE CLIMBING DOWN BACK OF LOADER HIS FOOT CAUGHT ON STEP, CAUSING HIM TO PUSH UP WITH HIS OPPOSITE LEG STRAINING HIS ABDOMEN WALL.

AFTER INSPECTING WASH PLANT SPRAY BARS, EE LOST HIS FOOTING AND CAUGHT HIMSELF WITH HIS LEFT HAND. THIS CAUSED SHOULDER AND UPPER ARM STRAIN.

CHANGING SCREENS ON PRIMARY SCREEN, WHEN EE FELLOFF OF TOP OF SCREEN AND LANDED ON WALKWAY ON HIS BACK.

WHILE CUTTING THE DRAGE CABLE ON THE DRAGLINE WITH AN OXY/ACET TORCH, SOME MOLTEN SLAG SPRANG UP FORM THE CABLE WHEN CUT AND WENT DOWN HIS GLOVE - BURNING HIS HAND.

WHILE CLEANING JAW CRUSHER GRATE, A ROCK CAME LOOSE AND FELL ON THE RIGHT HAND CAUSING 3RD FINGER TO BE HIT. THE EE NEEDED 4 STITCHES.

WHILE EXITING WATER TRUCK DRIVER HIT THE DRIVER'S SIDE DOOR HANDLE. THE HANDLE BROKE AND THE DRIVER CUT THE INSIDE OF HIS HAND.

WHILE REMOVING A PIECE OF OVERBURDEN FROM A JAW CRUSHER, THE OVERBURDEN BROKE AND A PIECE SMASH ED THE FINGER OF THE EE.

WHILE LOWERING TORCH TO GROUND FROM SCREEN CATWALK AREA SLIPPED AND FELL ON TO HAND RAIL. THE HAND RAIL BROKE AND EE FELL ON TO TAIL PULLY GUARD. FROM THERE FELL ONTO GROUND.

WHILE POURING CONCRETE EE WALKED IN WET CONCRETE WITHOUT PROPER SAFETY EQUIPMENT. CAUSING CHEMCIAL BURNS ON LEGS.

DOING MAINTENANCE ON L-JAY CRUSHER SWINGING SLEDGE HAMMER AND HIT FINGER BETWEEN WALL & HAMMER HANDLE.
