EE was squatting while monitoring hydraulic pump pressures. When EE stood up, EE became dizzy and passed out. EE fell, resulting in fractures.

Pinched little finger on right hand while shoveling out from under the crusher, doctor lanced finger nail to reduce pressure on nail. Employee reported injury to manager after being terminated due to reductions.

EE stated he slipped and fell off access ladder of front end loader. There was no lost time until June 10, 2008.
Employee was walking down side of a pile of overburden when his right ankle folded under his weight, breaking bone in ankle.

Accident was not reported until 07/05/06 @ 10:25am. While stepping off Excavator 346, slipped in slime on track. As I tried to catch myself, the door of excavator 346 slammed shut on right wrist.
Employee was working on conveyor support structure, and as he walked under conveyor it fell and hit him on the head. This report has already been turned in to the Local MSHA Office
EE was using cutting torch to cut a piece of metal. The metal was not secured and after it was cut it fell off the bench onto EE's right foot. EE had X-ray and returned to regular duties on 1/13/06. EE called in on 2/13/06, stating he was having problems with his foot. 2/13/06 would be the first day lost due to his injury. Release form is on file for 1/13/06.

Ladder slipped when climbing down off dragline,foot went between runners ,catching leg below the knee. Sweeling occurred, but no lost time. A knot came up on his leg, went to Doctor on 1/5/05, had it checked, and than on 1/18/05, doctor lanced knot to drain. Employee on light duty until 1/28/05
