IP stated was coming down a ladder and lost footing. Slipped and landed on feet. Reported foot pain.

Miner was cleaning under conveyor with a machine (Vermeer)- when lifting bucket, bucket hit another conveyor causing the machine (Vermeer) to kick forward and up pinching miners hand between Vermeer controls and bottom of conveyor.

Employee was removing the hose from the Hurricane 755 Industrial Vacuum, when EE was pulling on it the hose came lose the hose hit EE in the face causing EE to have to have 4 stitches above EE's left eye.

Employee was driving the JLG when they hit a pot hole causing them to bounce off the "safety foot peddle". This caused them to come to a sudden stop straining their back. Employee was brought in for evaluation and returned to work with restrictions, no lost time.

Miner was closing the doors on a Rotex machine. While pulling down on the latches for the door the miner felt pressure and pain in the shoulder and forearm.

A large piece of frozen sand caught on half on the belt and half off causing plugging. Employee removed the chunk of ice off the belt and placed on the ground. Employee did not feel any pain, but yet felt something that wasn't right in the groin area.

Miner was walking from one building to the adjacent building when miner lost EE's footing and slipped and fell. Miner was taken in to the emergency room due to pain in EE's left elbow area.
