EMPLOYEE WAS REMOVING COVER PLATE FROM SCRUBBER ON HIGHWALL MINER. HE LOST HIS GRIP ON THE PLATE, ALLOWING PLATE TO FALL ONTO HIS FINGER, AVULSING TIP OF FINGER (MIDDLE FINGER, LEFT HAND).
