WHILE LINING UP BOLTS ON A CONVEYOR BELT WIPER, A BOLT GOT STUCK AND EMPLOYEE TRIED TO HIT THE BOLT WITH A ROCK. THIS CAUSED THE WIPER TO BOUNCE AND PULL HIS LEFT HAND IN BETWEEN THE WIPER AND CONVEYOR BELT, SEVERELY CUTTING TWO FINGERS ANDTHUMB OF LEFT HAND.
