HE WAS INSTALLING ROCK BOLTS AND MATS IN 0210 STOPE. HE HAD DRILLED AND INSTALLED ONE BOLT AND DRILLED THE HOLE FOR THE SECOND BOLT. HE WAS USING THE DRILL WITH BOLT DRIVER TO HOLD UP THE MAT WHILE HE PLACED THE SECOND BOLT IN THE HOLE WHEN THE DRILL SLIPPED. HE WAS HOLDING ONTO THE AIR HOSE AND UNABLE TO CONTROL THE SWING OF DRILL. HE LET IT FALL AND IN THE PROCESS THE JACK LEG TU
