EMPLOYEE WAS ATTEMPTING TO SLIDE A STACKING CONVEYOR, CAUSING UNDUE STRESS ON A POINT NOT DESIGNED TO TAKE IT. IT COLLAPSED CRUSHING HIS LEFT HAND SEVERING IT IN TWO PLACES. BREAKING HIS ARM ( RIGHT) BREAKING HIS RIGHT LEG AND BREAKING HIS PELVIS.
