THE MINER WAS WORKING ON AN EXCAVATOR, WHILE REPLACING A HYDRAULIC HOSE AND WHILE LOOSENING THE HOSE, THE PRESSURE BUILT UP AND THE FLUID HIT THE MINER IN THE FACE AROUND THE LOWER LIP AND CHIN AREA.

EMPLOYEE WAS WORKING WITH A PIECE OF STEEL AND GRINDING WHEEL. EITHER A STEEL CHIP OR PIECE OF WHEEL GLANCED OFF RIM OF GLASSES AND INTO CORNER OF HIS EYE. WE FLUSHED THE EYE AND EVERY THING SEEMED ALRIGHT. BUT HE WAS BOTHERED LATER AT NIGHT AND WENT TO DOCTOR.

INJURED WAS OPERATING SCREEN PLANT WHEN DISCHARGE BELT BEGAN TO SLIP ON HEAD PULLEY, HE ATTEMPTED TO APPLY TUBE TYPE BELT DRESSING AT WHICH TIME HE BECAME ENTRAPPED IN HEAD PULLEY. ENGINE WAS SHUT OFF BY CHOKE SWITCH BY OPERATOR MACHINE WAS NOT GUARDED.
