EE WAS THREADING CONDUIT USING A RIGID 300 THREADER. EE FAILED TO LET THE MACHINE STOP ROTATING BEFORE ATTEMPTING TO REMOVE CONDUIT, CAUSING HIM TO JAM HIS FINGER BETWEEN THE ROTATING HEAD AND THE FRAME OF THE MACHINE.

EMPLOYEE WAS HELPING ANOTHER TO PLACE THE RAILROAD RAILS ON A HOPPER EMBED. ONE END OF THE RAILS FELL FROM ITS SUPPORT WHEN THEY WERE MOVING THE OTHER END AND THEY LANDED ON EMPLOYEE'S WRIST.
