FAILED TO MAKE SURE THE FEED BOX WAS SECURE TO THE LIFTING HOOK ON THE CRANE TRUCK BEFORE IT WAS MOVED. THIS CAUSED THE HOOK TO SLIP OFF KNOCKING BOTH MEN TO THE GROUND.
