Yavapai Apache Sand & Rock · Struck against a moving object

Employee was operating 631K Scraper. The Employee was going down the hill from the new strip and hit a dip which caused EE to lose EE's footing from the brake peddle which made the scraper accelerate speed going down the hill; when EE reached the bottom of the hill EE was able to stop the scraper but said EE had lower back pain.

Yavapai Apache Sand & Rock · Struck against a moving object

637G scraper tripping over burden next to 11' highwall, working to close to berm, over burden was soft clean dirt. Scraper slid slowly over edge sideways and slowly turned over. EE was ok but he was taken to urgent care to be check out. EE knew better than to operate equipment that close to berm, we have had many safety meetings about this. Tire tracks where way to close.

Employee operating scraper - hauling to crusher. Ran off the haul road onto rough embarkment causing him to be tossed from side to side in the cab.

WHEN EE WAS CLEANING THE GRIZZLY BARS ON THE VGF HE TURNED TO THE LEFT AND HIS RIGHT FOOT SLIPPED OFF THE GRIZZLEY BAR, HE THEN TRIED TO CATCH HIMSELF FROM FALLING WITH HIS RIGHT ARM IN DOIG S O PULLED TO DARSEL MUSCLE IN HIS NECK.
