While installing the front brake cooler it slipped off the mounts and smashed his left hand.

He was still feeling pain the next day and went back to the clinic and was told to take the weekend off. Employee was scheduled to work on Saturday. The Safety Dept. was not aware of the second visit or that he had missed a day of work.

EE CUT LEFT INDEX FINGER WHILE TRIMMING FLEX CONDUIT WITH RAZOR KNIFE.
EMPLOYEE WAS WALKING ON A BLAST PATTERN WHEN HE FELT A POP IN HIS RIGHT KNEE.
Employee was trying to move a boat when he felt a strain in his back. Employee went to the clinic and received no treatment. He followed up at the clinic on 01/11/05 and they gave him physical therapy because they saw no improvement. The injury, at that time, became a reportable.
Employee was prying copper cathode from a stainless steel blank and the copper struck the employee's wrist.
EE WAS STRIPPING COPPER WHEN BAR SLIPPED CAUSING FINGER TO SPLIT
Employee was harvesting in Central tankhouse with a strongback. As employee went to lock the strongback in place with his left hand he put his right hand down for support and a cathode fell 2 inches, hitting his index finger.
Employee removed indicator plate to look at the actuator and he noticed the shaft appeared to be protruding out. He touched the shaft and it fell down, cutting his upper lip.
Employee was in the process of installing a shelving unit. When the employee picked up a sheet of metal used as backing for the shelf it slipped out of his hand, lacerating his right thumb.
Employee was retrieving a 4 gallon container of sulfuric acid when the container slipped and fell. The container broke open and sulfuric acid splashed on his left knee. Initial diagnosis was for a first degree burn, however additional treatments have been required making injury reportable as of 09/29/04.
While changing the cutting edge on a rubbertire dozer, the operator received a laceration to his left hand.
The fire alarm and suppression system went off on the #64 shovel due to a fire in the RPC bank #2. Employee shut off power at the switchhouse and then entered the shovel to investigate electrical problems. There was a very small amount of smoke in the shovel and the smell of burned electrical components.
Employee was in the process of lowering a fuel tank to the ground. The tank slipped, approximately 1 ft. from the ground, and when it hit the ground the remaining gasoline in the tank splashed out of the filler tub onto employee's face.
Employee was moving the forks over when his hand slipped and he cut the heel of his hand on a sharp part of the mast.

Employee was installing a section of dipper handle racking when the weld broke loose causing the metal to rotate, striking employee in the head and face.
EE WAS TAKING OFF A 1 1/2 TON COME-A-LONG WHEN THE CHAIN FAILED. THE CHAIN HIT THE EE UNDER THE CHIN CAUSING HIS JAWS TO CLAMP SHUT CHIPPING HIS TEETH.
EMPLOYEE PUT HIS HAND IN HIS POCKET CUTTING HIS HAND ON A RAZOR KNIFE.
EE WAS CUTTING A FILTER MEDIA BAG WITH A LEATHERMAN AND CUT HIS THUMB ON THE BLADE.
Allergic reaction to latex gloves.
Employee was doing his regular tasks working at the Crush/Convey Division and became dehydrated due to heat.
EE BEGAN EXPERIENCING ACHING IN HER RIGHT WRIST APPROX. ONE MONTH AGO (2/25/04). SHE SAW HER DOCTOR ON 3/25/04. DIAGNOSIS WAS CARPAL TUNNEL SYNDROME. SHE WAS GIVEN A BRACE FOR RIGHT WRIST. DRUG & ALCOHOL SCREEN 4/1/04.
