On 3/24/2017, at approximately 1:45 am, a non-injury incident occurred. The operator of the C/N 620 Haul Truck was traveling on the haul road when ee had a personal medical condition that resulted in the truck overturning. This report is being filed under the direction of the local Mesa Field Office, per J. Summers, MSHA Inspector.

As the employee was trying to fit an oil seal ring (1/2 ring) onto the Ball Mill feed-end swivel bearing, it slipped from ee's grasp and fell approximately 24 inches landing on ee's left foot. The 1/2 ring weighs approximately 40lbs. The ring fractured ee's left big toe. The employee was wearing hard toe boots but the ring fell in just behind the hard toe.
EE was working in the control room of the VLE Pilot Plant monitoring the plant. A bee got in the control room, landed on employees hand and stung EE. Employee finished shift, reported for shift the next day and after shift EE called EE's supervisor and told supervisor of the bee sting and that EE's hand and arm had some swelling so supervisor advised EE to seek medical attention.
A diagnostic mechanic was moving toolbox when shoulder popped causing pain.
A powder employee was using an articulating loader to clean a blast pattern up for the blasting crew when the center pin broke causing the cab to tip backward. The operator experienced some jostling and received a minor injury to the neck. The employee returned to the clinic on 7/12/2017 and was prescribed pain medication.
An employee was in the LV712 transport van traveling to 12 shovel. When van hit a pot hole, employee experienced lower back pain. Employee traveled 1.5 miles to EE's assigned equipment at the 5150 before EE called HS3. On the evening of 7/4/2017, the employee returned to the clinic and was prescribed pain medication. Employee was scheduled off 7/6, 7/7, and 7/8.
EE was emptying a sulphur tray when EE felt pain in EE's back.
An employee was replacing liners when the platform shifted causing ee to lose balance. EE jumped up and landed on heel causing a fracture to the heel.
On 2/2/2017, an employee was exiting a vehicle and twisted ankle. EE did not report the event until 2/8/2017.
An employee was driving the truck on the haul road and hit a pothole causing the operator to feel pain in employee's lower back and neck.
On 1/12/2017 at approximately 4:00pm, an employee was checking fluid levels on a truck. The employee reported the following morning, on 1/13/2017, that they felt stiffness in their back.
After the employee changed out an oil line on the 3-B oil unit, employee proceeded to stand up. As employee was about half way up employee felt a pop in employee's left knee followed by severe pain.
An employee propped a step ladder against a vehicle and began to step up on the 1st step when the ladder slipped causing employee to fall approximately 1ft to ground and twist employee's ankle.
Employee was walking back to crew truck for lunch when EE stepped off a curb into dirt and sprained ankle.
At approximately 1950, a crusher trainee was preparing the #2 shorthead for maintenance by pulling back the dust curtain. While doing so, they tripped backwards and fell hitting the handrail. Their right arm was caught up in the rail and twisted the arm.
Employee was carrying a clip board, thermal temperature gun and radio in the right hand and using the left hand on the handrail. As the employee started climbing the second flight of stairs the employee felt a grind in the left knee causing instantaneous pain while stepping on the second step.
An employee was working on pipes when they mistakenly opened the line causing a small section of pipe to strike their arm and face.
The Shovel operator was loading the haul truck on right side while making second pass into bed, the dipper door opened up contacting the duck tail and causing the truck operator to feel pain in EE's neck.
On 3/20/2017, an employee was cutting samples. When EE attempted to move the last plate, it slipped and landed on the top of EE's foot. On 3/30/2017, the employee returned to the clinic and x-rays revealed EE sustained a fracture to the top of right foot.
On 3/9/2017, an employee was getting out of car before shift and rolled ankle. EE attended a follow-up appointment on 3/10/2017 was given medications that prevented from returning to work on 3/11/2017.
On 2/17/2017, an employee was walking and EE rolled ankle. On 2/20/2017, the employee was given restrictions.
Employee was going down the stairs of the lube truck; EE lost grip, slipped and landed on knee as EE turned to put foot on the step. The following morning the employee was given restrictions.
Employee was pushing a cart of scaffold material into an elevator. the long vertical riser hit the back of the elevator causing the load to shift which pinched the EE finger between parts
Walking down the stairs on the last step employee's knee popped
Employee was changing out skirting on a conveyor belt. EE pulled on the skirting and some dust material came down from the feeder belt above EE. Employee felt discomfort in right eye.
An employee knelt down and felt pain in knee.
An employee was hanging cables when they lost their footing and fell backwards on the ground injuring their shoulder and wrist.
Team was working on conveyor belt system vulcanizing belt at Morenci Metcalf Mill Building. Cart used to transport Belt Press bars. Belt Press bar fell approximately 2 feet off from the cart and struck Employees left foot/ankle.
The employee was walking slowly down the stairs right after a meeting at the auditorium. EE slipped and fell even though EE had three points contact. EE aggravated a previous injury as a result.
While employee was assisting with bucket samples, employee tripped over the corner of a rubber mat. Employee fell into the back of their coworker and fell to the ground. During fall, employee made contact with a yellow pole and impacted the ground. Employee felt pain to the left shoulder.
Employee stepped over handrail to access an idler and when employee stepped down on the walkway employee rolled employee's ankle on a small rock
An employee was walking out of the building and bumped employee's head on the apex of the building causing pain in employee's head and neck.
On 8/31/2017, an employee was spraying down the area with a water hose and felt pain in shoulder and upper back. On 9/1/2017, employee reported the injury and was medically evaluated and released to work. On 9/14/2017, the employee returned to the clinic with continued pain and was given restrictions.
An employee felt pain in their neck while the truck was being loaded at the shovel.
An employee stepped on an air hose and stumbled. Employee reached out to catch their fall, pushing their hand against an pillar block causing a fracture to the hand.
On 8/22/2017, the operator of the 555 Haul Truck had stopped in the road. Another truck attempted to stop and slid into the back of the stopped truck at a slow rate of speed. On 8/27/2017, the employee reported that EE was experiencing pain in neck.
On 8/14/2017, an employee was lifting a haul truck hood when back popped causing pain. On 8/23/2017, the employee returned to the clinic with increased and continual pain and was given restrictions.
On 8/14/2017 an employee picked up a 20 pound sledge hammer and felt a pop in shoulder. On 8/17/2017, the employee returned to the clinic with increased pain and was given restrictions.
A Drill-Helper was attempting to load another stick of drill pipe into the Pipe-Arm Loader on drill rig #418. The Drill-Helper was holding a 36"" pipe wrench with R hand & operating the remote control with L hand, while dragging the pipe into the cup the R pinkie finger was caught between the pipe wrench & hydraulic ram cylinder causing an avulsion to underside of pinkie.
Employee stepped in a soft spot of sand while walking and felt pain in their right knee.
Employee descended a ladder (face first) and felt pain in right shoulder.
Employee experienced pain in ee neck after equipment. On 7/11/17 the employee was given restrictions but started ee 7 days off.
A drill operator was climbing the steps on the 11R drill when knee buckled causing ee to hit the same knee on the steps. EE received a minor scrape and some knee pain as a result. On 7/7/2017, the employee returned to the clinic and given restrictions.
Employee was trying to pry off copper sheet that was still attached at the bottom of the blank. EE put pry bar through the top of the sheet and went forward with force and when the copper sheet released it put the employee in the line of fire for a pinch point. EE pinched 4th finger of left hand between the sheet and the pry bar.
An employee was walking in the parking lot and tripped on gravel, twisting right ankle.
EE failed to disconnect the 4 inch hose causing to be sprayed with sulphur on legs.
Employee was working on the discharge bearing of the ball mill when EE's left thumb was caught between the swivel and the bearing cap. The employee returned to the clinic on 4/24/2017 and was given restrictions.
Employee was getting loaded at the 46 shovel. When the first dipper was put in the truck employee experienced some jarring in the seat. The employee was seen at the clinic and released to return to work. On 4/10/2017, employee returned with continued pain and was given restrictions and returned to next regular schedule on 4/13/2017.
EE was raising the gantry when the gantry fell and pulled the EE's arm.
An employee was using a pneumatic hammer to remove liner bolts and accidentally activated the hammer and it struck employee's finger causing a fracture.
An employee was repairing a flat tire and felt pain in right lower arm.
An employee reached to for coffee creamer in the refrigerator door and scratched eye on a zip lock bag.

Employee was trying to loosen the bolts on the cyclone using two hand held wrenches when the tools slipped and pinched the webbing on the palm below the index finger resulting in a 1/2 inch laceration. Employee was wearing leather gloves and all PPE.
An employee felt irritation in their eye after they rinsed their face and wiped it with a paper towel.
An employee was using a pry-bar to remove a chunk of locking compound when hand slipped and was pinched between the bar and the brass fitting causing a small laceration.
An employee was performing a pre-shift inspection when employee placed hand on the stiff arm of the dozer and received a laceration to the hand from a metal burr.
An employee returned back to original work area and tripped over an H-Beam at floor level. Employee fell forward and hit the concrete floor. Employee bruised face, hand and the left shin. The next morning, 8/18/2017, the radiology report revealed a fracture to the nose.
A employee was torqueing lug nuts on the inner wheel of a haul truck tire. EE accidentally squeezed the trigger and EE's finger was pinched between the socket and lug nut causing a laceration that required multiple sutures.
A beam trolley rolled off the beam, striking an employee on the hand below.
Injured EE & helper were putting cathodes on strongback with the cathode hand tools. When the side that the helper was moving slipped from the strongback causing the cathode to fall on the injured EE's side, crushing finger in between the hand tool & the transfer case. EE's supervisor took EE to the clinic where EE received 3 stitches to tip of middle finger on left hand.
An employee was tightening a hose fitting when the wrench slipped allowing their arm to contact a support for the fan shroud causing a superficial cut on their forearm. The physician applied a steri-strip(r) to keep the wound clean, dry and to avoid infection and not for wound closure.
Employee was exiting the Guard Shack, and experienced a muscle cramp in left knee, then rolled left ankle and fell to the ground.

Employee was at the 5280 fuel dock fueling the 598 HT around noon, was waiting outside of truck to finish fueling. Wind was blowing and after a couple of hours started to feel irritation in left eye, having pain and eye watering. Employee was given antibiotic drops for preventative measures.
Employee was walking to clock in and passed a group of people smoking, someone flicked a cigarette and that's when the employee felt ashes in left eye.
An employee was wearing their personal gloves while driving and rubbed their left eye. Later, started experiencing itching, burning and developed redness. Employee was taken to clinic for evaluation. After being evaluated employee was found to have a small Corneal Abrasion. Employee was allowed to return to work
On 3/14/2014, an employee took gloves off to wipe the sweat of their brow and continued to work. After an hour of work employee felt irritation to left eye and flushed eye for 15 minutes. Employee was seen at the clinic and no treatment was given. On 4/11/2017, the employee reported they continued to experience discomfort and was given antibiotics to prevent infection.
An employee was removing blanks and received a laceration to the lower arm requiring 5 sutures.
EE ate lunch in the office then rubbed eyes and reported there was something in eye.
An employee was using an 18"" pry bar to pull the gear box shaft down when the bar slipped and struck employee's upper lip causing a cut.

Employee was cleaning at the Stargo Scrubbers when the material splashed up seeping through ee Tyvex causing skin irritation
An employee was conducting their pre-shift on the truck and hit the test button for the fire suppression system. When the test was conducted for unknown reason the system activated and they exited the cab, getting some of the suppression in their eyes. They experienced irritation of eyes and was given drops of ointment in each eye.
