While operating a haul truck, employee states he received a rough load and 3 hours later experienced neck/shoulder discomfort.
Employee was stripping copper blanks at the 2 stripping machine when he felt a pain in his lower back. Employee then fell back onto the floor grating.
Employee fell from behind cab area while performing repairs on the haul truck. (BED WAS UP; FELL OFF SIDE OF TRUCK (APPROX. 12' TO GROUND), HITTING HEAD ON TIRE OR FUEL TANK AS HE FELL. FRACTURED TEETH, LACERATED LIP, POSS. DISPLACED PALATE. NO OTHER INJURIES.)
Employee got on top of the engine compartment of the #30 crawler to look over the dump. He tripped on the air cleaner, causing him to grab onto the exhaust pipe, burning the palms of both hands.
Operator was being loaded at a shovel when a rock fell off of the shovel dipper and struck his truck.
Employee was removing bolts with a 1/2 inch drive long handled ratchet and, while pullling on the ratchet in an awkward position, strained his lower back.

Employee got out of his service truck and started to put on his coveralls. When he raised his leg his lower back spasmed and he fell to the ground.
Employee was under a haul truck bed that was mounted on stands. He was cutting a piece of metal. The piece of metal broke free from the haul truck bed and struck employee's left foot.

The socket came apart-striking his left thumb causing a laceration and a fracture of the thumb bone. He received stitches and a splint.
Employee climbed into bottom of crusher to inspect it before doing a PM. He noticed some rocks in the upper part of the crusher and started to climb out. When he did some rocks fell, hitting employee's hand.
Employee was the driver of the SX155 Vactor truck. Employee stated the brakes failed as they were coming down a small hill. He turned the vehcile to the right, causing the vehicle to roll over.
Employee was lifting a 12"" stainless steel flange off of his truck when he felt a pain in his right groin area. per company call 3-25-05, the hernia was repaired. coder 12
Electrician strained his lower back while helping to lift a 480 volt disconnect on November 29, 2005. The employee did not seek medical attention until December 19, 2005.

Employee was climbing onto the #80 motorgrader. When he pulled on the door handle to open it, it swung open causing him to lose his balance and his grip on the door handle. He held onto the motorgrader with his left arm, supporting his entire weight.

A heat gun used to shrink tube when building cables was set aside and a person touched it with his forearm. Heat gun was off but its tip was hot.

Employee was removing boxes of assay pulps from a shelf in the Geology main core storage building. She was standing on a portable staircase receiving boxes from another employee working above. After receiving the box, she started to climb back down and lost her footing, resulting in a fall.
Employee was working on a plugged chute. As the employee pushed the pry bar into the hard material, the bar went to the side and pinched the employee's right hand, causing a cut.
