Employee was connecting iron frame work on a structure when he felt a pain in the groin area. He reported it on 1/12/07.
Employee stepped off a bus and felt a sharp pain in his right foot. Employee continued to work and stepped on a rock causing more pain.
Employee's shift began at 7:00 p.m. on 12/7/07. Employee was operating #28 RTD cleaning the left side of the #14 shovel. The #14 shovel turned to load a truck on its right side. The counterweight flattened the cab of the dozer. Employee was rescued from rubber-tired dozer.
Employee was loaded at the #13 shovel pit and was stuck in the mud. Employee dumped the load and the front tires of the haul truck went up into the air and then back down again to the ground jarring the employee. EMPLOYEE DID NOT REPORT THIS INCIDENT UNTIL 12/17/2007.
Employee was operating the #553 haul truck and slid into the back of the #518 haul truck.
Employee's shift began on 11/18/07 at 7:00 p.m. Employee was operating a haul truck and halfway through his shift began to experience discomfort in his back.
Employee was operating Haul Truck #529, being loaded by the #10 shovel. On the 3rd dipper, a large rock was loaded into the truck. The boulder landed on the truck canopy, causing the employee to be jarred. After the incident, the employee was released to work. She was re-evaluated at the Clinic on 12-5-07 and given restrictions not allowing her to return to work.
Employee was a passenger in the rear seat of a van. Driving along, the van hit a pot hole in the road causing the employee to strike her head on the roof of the van.
Employee was at the north stacker taking a cable off of a dozer. He bent down and felt his left knee pop.
Employee was loading a bag of media used in the grinding process and slipped on ceramic beads on the floor. Employee put his left arm down to break his fall and fractured his wrist.

Employee was cleaning out a feeder pit. He was climbing out, struck his head on the vacuum truck connection which caused him to fall backwards onto the roller of the #6 belt.
EE was operating the #537 haul truck being loaded by the #13 shovel. The shovel dipper and the haul truck made contact causing him to experience back pain. Employee had just finished a drill run then put his hand below the swivel on the pipe to feel if the core spring was gripping, (it did not grip), he then lowered the drill string. There was slack in the line, a moment later the core slipped back onto the tube, causing the string to drop and crushed his index finger between the bottom of the swivel and the top of the head.
EE was overseeing a job on a shovel. He bent down & when he tried to stand up, his back locked up on him. The validity of this claim was questioned, 7000-1 was not submitted. Employee was helping crane men unlock strongback on floor when he noticed a anode was not properly in the saddle and decided to get on rack to fix anode and doing so he fell off rack.

Employee was attempting to pull a piece of four inch pipe on the stockpile when the pipe got caught between drip and other pieces of four inch. The employee was then thrown off the ATV which then fell over on the top him.

Employee was climbing down from the bed of the CC-152 boom truck when he started descending the boom truck his right foot slipped off the rung, causing his right leg to hang up between the rungs on the dismounting ladder.
Employee was working under the pan feeder of Crusher 2 clearing rock off a concrete ledge so a skid steer loader could scrape the material out of the pocket. The skid steer scraper attachment made contact with her left ankle causing a laceration.
Employee was stung by a bee as she walked to the front gate. The bite site of right arm got infected and was treated. Ongoing.
While tightening fitting on a hydraulic hose on the 536, the adjustable wrench that the employee was using slipped causing him to hit his body under his right armpit. He felt a sharp pain.

Employee's shift started at 6:00 p.m. on 12/28/2007. Employee was using an air ratchet to mount a tire on a haul truck. The ratchet came loose off of a lug nut causing the employee's arm to twist backwards.

Employee was dismounting a haul truck and was halfway down the ladder when she caught her elbow on the grill. Employee's left foot slid between the side dismounting rails.
Employee was jarred in a vehicle when the vehicle hit a pot hole in the road.
EE initially injured her arm on 5-16-07 & was released full duty on 8-10-07. She was working regular job; however, on 9-19-07 She complained of pain and was seen at clinic. She was released with restrictions. She worked one week under restrictions, complained of additional pain and was re-evaluated at clinic. She was put off work on 9-26-07.
Employee was hosing down under the #5 West ball mill. Employee was stepping in mud. Employee attempted to step backwards; however, his left foot was stuck in mud. Employee fell backwards straining his left knee.
Employee was working on the north MFL pad extending air lines on a 26 foot slope. Employee was positioned in the middle of the slope when she lost her footing and fell to the ground. Employee put her left arm out to break her fall landing on her left hand.
Employee was climbing out of the primary crusher when the ladder he was climbing slipped. Employee rode the ladder down approximately 4 feet before the ladder stopped.

Employee was shoveling muck at the R1A-R1B transfer tower. She twisted and felt a pop in her lower back.
Employee was existing a service truck. Employee stepped out on the running board and went to pivot on his right foot when his body twisted around. Employee felt a pop in his right knee. We were notified on 9/4/2007 that our insurance carrier has accepted this claim.
Employee was straightening drip line that was being pulled by an ATV. The drip line got caught on a boulder. Employee lifted the drip line over the boulder at the same time the ATV was pulling it, causing the employees arm to get jerked.
Employee was operating a skid steer loader with a scraper attachment. She was back dragging fine crushed material when the scraper blade caught on some structure and brought her to a stop. The abrupt stop resulted in pain to the neck and shoulder area.
Employee was pulling on rubber belting to straighten it when he felt a pop in his groin.
Employee was loading pulley onto truck using forklift when pulley rolled off of forks and smashed employee's fingers between bearing housing and torch box.

Employee was chipping on a truck bed in the Fab Shop. Employee was wearing glasses and a faceshield when a piece of hot slag went into his right eye.
Employee was operating a haul truck and after the shovel operator loaded the third dipper, employee felt pain.
Employee was lowering herself off the 17R drill. She caught her foot on the bottom rung, fell, and landed on her left arm.
Employee's shift began at 6:00 p.m. 9/20/2007. He got out of the cab of his haul truck. His foot got caught under a brace bar on the truck causing him to fall and twist his body.
The employee was setting up cable rigging with 1"" cable and attaching a 1 1/4"" shackel. The weight of the cable rigging caused a pain in the lower back of the employee when he stood up.
He was moving in electrical cabinet with 3 people. When they got to a tight space he tried to move the cabinet alone and strained his lower back.
Employee was working on the #533 haul truck drive line and smashed his left hand.
EE was grinding surface of Moly flotation cell & mechanic tried to move grinder wheel guard before grinder came to a complete stop. Glove got caught in grinding wheel guard cutting left index finger at 1st knuckle. Transported to clinic, cut was cleaned & bandaged. Restricted to working in clean environment. Keflex, ultram prescribed, full release no restrictions 8/27/07.
Employee was retrieving solids behind the #16 ball mill. Employee stepped over a hose and her left leg fell through a metal grating. Employee grabbed railing with her hands to stop the fall and struck her left knee on the metal grating.
Employee climbed off the #47 blade to speak to another employee. Employee began to climb back up on the blade and felt a muscle pull.
Employees were lowering vertical motors onto crates. While lining up the bolt, the motor slipped and caught the injured's middle left finger between the bolt and the deck of the truck.
Employee stated: using a jack hammer on 8/1/07 and woke up on 8/2/07 with his right hand swollen. Then reported to foreman. Employee did not request medical attention until 8/9/07.

Shift began at 7:00 p.m. on 7-22-07. Employee left a shovel in the Garfield area, headed south to the UCC dump, when the front left tire blew out. Employee was rocked in the haul truck when the tire blew.
Employee's shift started on July 12, 2007 @ 7:00 p.m. Employee was cleaning up around the #2 shorthead crusher. Employee had loaded a wheelbarrow and was going to dump it into the crusher when the wheelbarrow tipped over causing the employee to fall. Employee twisted her right knee when she fell.
Employee was standing to the side of the #2 adapter and was using a 16 lb. sledge hammer to adjust it. When employee swung the sledge hammer and it struck the adapter, employee felt a pop in his left elbow.
Employee's shift started on 7/5/07 at 7:00 p.m. Employee was operating the #548 haul truck and was being loaded at the #70 shovel. After the first dipper, the employee experienced back/neck pain.
Employee lacerated right middle finger pulling on a control arm.
Employee was in the back of a truck handing out face shields. Employee placed her right foot on the tire to climb down and lost her grip. Employee landed wrong twisting her left leg.
Employee was operating the #553 haul truck and was being loaded at the #10 shovel. On the first dipper pass there was a boulder in the dipper. It was dropped into the bed of the haul truck causing the employee to receive a back/neck strain.
Employee was drilling a 1/4"" hole with a 1/2"" drill, when the bit stuck, causing the drill to rotate 360 degrees. He has a hairline fracture to his right ring finger.
While moving snub pulley away from structure to place shims, one of the chains that was being used to pull the pulley away came free and struck the employee on the right side of his rib cage.
Employee was using a 4"" grinder equipped with a mataubo blade cutting up 4"" galvanized 'C' channel. The bench had an accumulation of dust on it. A gust of wind blew dust up under his face shield into his right eye.
The EE was working on throttle linkage inside cab of d8l dozer. Removed floor cover to access linkage and standing down inside opening. Left foot slipped causing ankle to twist. EE put on miner restrictions & given otc ibuprofen & ice. Also give crutches if needed.

Employee was gathering a 4x4x20 block to set jack for the C-drum trunion roller replacement. He was in fromt of the stacker repair bay loading the block from the CC-77 to the CC-133 truck to transport to the C-drum. While lifting the bar on to the truck in a twisting motion, he felt pain in his upper back. Employee was transported to the clinic.
Employee was leaving the office from a safety meeting. When he opened the office door he grabed the door frame and reached back into the office to grab his radio. The door closed pinching his right hand fingers.
Employee was picking up a Drisco pipe die when he felt his right index finger pop.
Employee was in the process of retrieving a rope, that had been thrown over the bank, for cable removal. He came up from the bottom of the bank and when he pulled on the rope rocks came down from the bank and hit him. The employee lost his balance and fell causing injury to his shoulder. He was taken to the clinic.
Employee using a exit ladder which was up in retracted position on the side of a new P & H Shovel when the weight of the employee caused the ladder to extend down approximately 8 feet causing sprain to his left ankle. Employee was placed on restricted duty.
EE was installing conductors into conduit. He was pulling in a downward motion from above his head to below his waist pushing conductors into the conduit. This motion exerts force int he elbows and shoulders. The strain/sprain is in both elbows where the forearm and tricep ligaments attach. (EE was placed on 4 days restricted duty: he had personal days scheduled prior to the
Employee was climbing down the ladder of his haul truck when his foot slipped. He did not feel any pain at the time and went home to sleep. When he awoke his lower back was hurting.
Employee was stripping wire with a razor knife. The knife slipped and he cut his left hand.
Employee was pulling electrical wire up on cable tray on conveyor. The rope they were using to pull with came undone. The cable fell down and struck him on his hand causing a hairline fracture to his right index finger.
Steel plate was pressed down and tacked in place. Employee then took position on top of plate to continue welding. While sitting on plate performing job duties, the tacks failed causing a sudden release of stored energy jarring employees lower back. EMPLOEE DID NOT START FEELING PAIN UNTIL 2/20/07 AND REQUESTED TO BE TAKEN TO THE CLINIC.
Employee was pulling 3a shorthead crusher and while exiting the stairs by the 2b shorthead he turned at bottom of stairs at ground level and stepped on pin on step bearing plate causing sprain to right ankle. He was looking up towards crane at time of incident.
During shift change Employee was moving toward the back of the 140-0 van to be seated. The individual driving the van began to move while employee was still upright causing them to fall. The driver of the van didnt stop to make sure employee was in the seat.
Mechanic was moving a bottle of oxygen and lost his footing. He fell backward onto a piece of angle iron directly onto his buttocks.
The operator of the 520 HT was turning around on the 5700 L Dump. The steering wheel whipped back around catching the operator's left thumb pulling it back.
Employee was at the SX Central working overtime on the SU-33 water truck. He exited the driver's cab and stepped down and his left ankle twisted/popped on him.
The cable crew laborers were hanging bridges on the ramp leaving the 13 shovel. As the Laborer was attempting to hold the rope while it was to be tied off, the rope started to slip. The laborer let go and started to run when injury to knee ocurred.
Was cleaning and cutting concrete anchor bolts with a cutting torch. When he stood up and stepped backwards, he tripped on a concrete peir and fell. When he fell over backwards he caught himself with his right hand causing a hairline fracture.
While burning the fastners off the hold down bracket on the thrust block on a 12 line for the Tailings Dam at the 7-C deep well south of silver basin, an employee was adjusting his torch after lighting it when it slipped and burned the top of his right hand under the glove.
While employee was assisting the operator of the forklift, they were in the process of lifting a gear using a lifting sling. Both employees were focusing on the position of the forks and the sling and did not notice the position of the legs and the wheels of the extenda-fork forklift when it came in to contact with one of the employees ankles.
EE was cutting the gear drive base plate off of a classifier at the #14 ball mill. When he stood up he placed his hand on the track for the ball cart to steady himself. The ball cart came by as soon as he placed his hand on the track clipping his r.h. index & middle fingertips causing his nail to pull loose and resulting in a stitch to keep his nail in place.
Employee was driving a railroad spike, when spike popped out and hit his right cheek causing a laceration requiring sutures.
Employee was moving and stacking concrete forms when he smashed his left ring finger with one of the forms. He did not seek medical attention until 12/19/07 and was required to wear a splint for 1-2 days after.
Employee states that one of the retract flippers from a TH-75 drill rig broke loose and landed on top of his hard hat. The pressure from the flipper caused his scalp to move downward causing his scalp to split open.
Employee was replacing a door frame and dust got in his eye.
Employee was cutting a piece of rubber belting with a utility knife. The knife slipped through the rubber causing a laceration on the employee's wrist.
Employee was driving forklift, forks were low and hit hard subject on ground forcing employee to go forward and hit chin on steering wheel handle.
Employee's shift began on 11/11/07. Employee was putting the cable sling ring on the crane hook when his finger got caught between the hook and the ring causing a laceration.
Employee was working in a manlift taking measurements on a dipper when he was exposed to welding fumes/smoke.

Employee was removing a cover from an electrical cabinet with a ratchet. The ratchet slipped off and struck the employee on the upper lip causing a laceration.
Cutting coolant hose with razor blade, blade broke causing him to jerk and strike himself in the right calf just below the knee.
The injured employee was holding the wear plate on the forks about three inches off the ground. The wear plate shifted causing wear plate to tilt forward pinching employee's three finger between the wear plate and the ground. Employee received a cut to right middle finger and was sent to the doctor and received 4 stitches
Employee's shift began on 10/14/2007 at 5:00 p.m. Employee was tightening the skirting on a conveyor when the wrench slipped and struck him in the face. Employee received a small laceration on his lip and a broken tooth.
Employee was transporting an oxygen cylinder from storage area and lost his footing causing him to fall onto a piece of sheet metal, causing laceration to left ear which required sutures.
Employee's shift began on 10/2/07 at 7:00 p.m. Employee was changing a drill bit on the 53R drill. As he was lowering the deck bushing with the remote control winch, he got his finger caught between the deck bushing and the deck wrench.
EE was stepping off back of van. He stepped on 2""-3"" offset in the concrete and fell backwards and tried to break his fall with his hands.
EE was moving a piece of stainless steel pipe and strained his groin. He was released to full duty the same day. Over the next 3 1/2 weeks he restrained it and became a hernia requiring minor surgery w/sutures and was released back to full duty.
Employee was cleaning under a cell line when she slipped and fell on mist balls. Employee received a laceration to her knee.
Employee was tightening a bolt on the pump drive when his wrench slipped hitting the back of his hand on the yoke causing about a 1 inch cut.

Employee was standing on the bed of the FM27 truck installing a welding machine. Employee stepped back and fell off the truck striking his head and shoulder.
Employee was welding on day shift. Employee states that his eyes felt tired when he went home. Employee experienced discomfort in his eyes the next day.

Employee struck a piece of angle iron with a hammer to break it off of a shovel boom. The angle iron swung around and struck the employee in the mouth, chipping a tooth. He saw a dentist on August 31st and was told that the chipped tooth must be repaired to prevent nerve damage.
The crew were making an add and when the driller lifted the pipe off the truck, the employee grabbed the pipe and had it in position while it was being lowered. He lost his footing causing the pipe to fall back against the mast slide smashing his finger between the pipe and the slide.
Employee's shift began on 8/6/2007 at 6:00 p.m. Employee was welding segments on the lip of a dipper. At the end of shift when the employee was washing up, he saw something in his right eye.
Tighting valve on saddle, felt a eye irritation with left eye, took off glasses and gloves rubbed eye, washed out eye and went back to work. Woke up next morning 8/7/07 with swollen upper left eye lid. Reported injury to foreman on 8/7/07.
Employee was in the stainless steel bin located in Salvage Yard #3. Employee was attempting to remove a piece of pipe from a pile when his right forearm came into contact with another piece of steel pipe. Employee received a laceration.
Employee was removing a dipper door. After inserting a 4x4 block of wood between the door and the dipper, the door shifted creating pressure on the wooden block. The block popped out and struck the employee in the mouth.
Employee was hammering a track pin with a 4-lb. hammer. A piece of pin flew off and struck the employee in his left leg.
Employee was removing front spider on 793 haul truck main driveline. He pinned finger between impact gun and torque converter when driveline bolt broke loose.
Employee was working on the engine swing on Truck 562. As he was walking up the stairs at the front of the truck, the a/c condensor core bracket was sticking out in the pathway, as he was walking up the bracket cut through his clothes cutting his right shoulder blade. After calling Triage nurse, employee was taken to the Morenci PD clinic.
Employee was using a hand grinder when the grinding wheel came apart and struck his right thigh causing a laceration that required sutures.
Employee was pulling on a steel plate and a spot on the plate was worn sharp. Employee lacerated the middle finger on his left hand.
Employee was attempting to place area cap onto a rebar that was too large. Her hand slipped and left pinky finger was fractured on rebar.
Employee was working on a hoist cylinder, hammering a wedge to allow a spacer behind the cylinder. When hammering the wedge, employee missed the wedge and struck his left index finger, causing a laceration.
Employee hit the side of a cutting edge with a 4lb hammer which caused it to splinter resulting in injury while working on the #84/24H motorgrader.
Employee was unplugging the crusher when a rock dislodged and struck the employee on the right forearm causing a laceration.
EE was lifting pipe equipment with forklift and needed the chain on leg adjusted. He did not get help. He got off of forklift and tried to reposition the chain, when the roller rolled off the forks and pulled the chain, pinching his finger between the chain and the steel leg.
Apprentice and Journeyman were climbing pole and while hanging a light Apprentice was going to move a little. He stuck left gaff into right leg in calf area requiring stitches.
Employee was chipping his weld when a piece of slag went between his respirator and safety glasses causing abrasion.
The operator of the 557HT was leaving the King 6200 dump when his truck started to slide into oncoming traffic. He turned his truck away from the traffic and as he did he hit a dry spot on the road causing his truck to go up on the left side berm. He was bounced out of his seat hitting his head on the ceiling of the cab. He was not wearing his seatbelt.
Employee noticed no lime being injected to 17 Ball mill he wiggled the rubber hose at the end of the injection point and at this point the lime actuated striking the funnel and sprayed him on the face.
Employee was removing a rock from a haul truck tire when the truck engine went into high idle and kicked up dirt which blew into employee's eye.
The 546 haul truck was loaded out by the 12 shovel and began to travel uphill loaded. While traveling on the ramp below the Garfield water stand, the 546 haul truck operator felt the truck jerk and then realized the haul truck was up on the berm.
The operator of the T432 road maintenance haul truck was dumping out stemming in western copper. Noticed flames coming out of the deck of the engine comparment and shut the truck down. Set off the fire suppresion in the cab to stop the fire, and the cab filled with suppresion which the operator inhaled a little.
Employee was with another employee checking the P-8 conveyor heading up the towards the head pulley when a gust of wind blew by, that's when the employee complained of something in his right eye. Employee flushed his eye but still had some discomfort and was taken to the clinic by his supervisor.
Employee was grinding on a verical metal weld in an overhead position. As he finished the first weld, he looked upward and as he did so, he felt dust fall into his right eye.
Employee was removing conduit in the central tank house and some airborne sulfate dust got into his eyes. Employee was taken to local clinic and was given a prescription for his eyes and released to full duty.
Employee noticed that a valve on cell 20 was not close all the way. He went back to re close the inlet valve on cell G 20 when he splashed electrolyte that was on the floor and electrolyte got on his clothing and saturated them and electrolyte got on his right calf on his right leg.
Employee was trying to take off a bolt that was stripped. Employee used a prybar to secure the bolt to prevent spinning. The bar slipped when the impact was applied and employee caught his thumb with the spinning bolt causing a laceration to his right thumb.
Employee was operating a dozer, another employee was operating an end dump and was given the signal to dump by the spotter. The spotter then realized that the dozer had moved but the material was already falling on the dozer. A rock landed on the dozer operator's hand, causing a laceration.
Employee was removing used ripper points from the back of his service truck. After tossing the next to last ripper point and reaching for the last ripper point, the employee felt something impact his left tricep. He did not see anything and continued to reach for the last ripper point when he noticed blood trickling down his arm.
Employee was driving water truck w/air seat, truck went over bump and employee's hard hat hit roof of cab causing him to bite down hard and fracture tooth
Drill string hung up and then dropped, smashing employee's right thumb between water swivel and chuck jaws.
The operator of the 564 HT was backed up in a trench in an attempt to dump his truck. After dumping out the operator pulled out of the trench and in the process the front of his truck came off the ground. When the truck descended it jarred the operator.
At the truck shop in front of pad 14 while dismounting the operator of the T501 missed the bottom step and fell backwards making contact,on the cement pad cutting his head.
While spraying at the A1A splitter, the employee was struck by muck and water in the face when co-worker sprayed toward him. Employee was taken to clinic and treated.
EE called the 402 and said that she hit a pot hole on the way to the 10 shovel. She was having pain in her neck and back.
While dismounting the 511 HT for a bathroom break, the operator appears to have pulled/stretched his groin area.
The operator of the 65 shovel was loading the T557 and on the third dipper, the dipper clipped the duck tail.
Employee was walking out from under cell line after observing work that was being done under A- cell line when a foreign object fell into employee's left eye.
Employee was working underneath a pickup and foreign matter dropped into right eye. Went to doctor on 1/9/08, however did not receive doctor's notes indicating prescription medication was given to him until 1/22/08.
Employee was attempting to close a door. The door stop was broken, so employee had to bend over to lift the door stop. While employee was bent over, door closed on employee's head.
