Accident type, without injuries

Savage received rail cars that were bigger and that were only 3/4 of the way full with Sulphur. That allowed the head space in the rail car to become greater and fill with H2S gas.

Employee was getting off 345BL Excavator when he noticed smoke coming from under the engine compartment. Employee shut off engine and grabbed fire extinguisher and attempted to extinguish fire but was unsuccessful. He stepped back and called for help. Excavator burned until help arrived.

Employee slipped and fell and his right hand was caught.

Crew member twisted knee while pulling sling from drill collars, employee was taken to medical center for evaluation.

Absorption of radiations, caustics, toxic and noxious substances

Employee was changing a leaking valve and was sprayed with acid mid after bumping a different valve on a separate line. Mist sprayed between overlaps in employee's PPE causing chemical burns to his waist and wrist.

Employee felt back pain while driving the 537 haul truck. This incident occurred on 09/08/2012 night shift.

Employee felt discomfort in lower back after being loaded at the 17 Shovel. Evaluated at Gila Health Resources on 7/12/2012, classified as First Aid. On 7/17/2012 employee was re-evaluated by Gila Health Resources and incident was changed to Lost Time.

Over-exertion in pulling or pushing objects

After our investigation and doctor reviews, it has been determined that the employee injured his right knee while pulling power cable. The employee was working his normal job until 6/15/2012 when he had surgery on his knee.

Employee was climbing back into the 86 dozer, when he felt and heard a pop in his right upper leg (back). Employee tore his hamstring.

The 617 haul truck operator felt back pain while being loaded at the 17 shovel.

Employee lost balance and fell, using his left arm to catch himself.

The PVC pipe glue melted and came apart spraying the employee with hot water causing second degree burns to the upper arm and leg.

Mechanic was swinging a 16 pound hammer, when he felt a pop in his left shoulder.

After an intensive medical & incident investigation proving this injury was non work related, an Independent Medical Evaluator examined EE's injury and concluded on 8/20/12 EE's right knee has a meniscus tear. EE complained of knee pain while driving an excavator. Investigation proved equipment inspected & working correctly, task was safely done per JHA. No witnesses.

Fall down stairs

Employee was descending the No. 3 apron feeder, when the wooden step that he stepped on wobbled. The employee twisted and his right knee buckled and popped and he fell to the ground.

A piece of fly metal embedded in the employee's right arm.

After our investigation, it has been determined that the employee injured his left arm, when he slipped and fell.

Contractor employee was using a shovel to clean up ore from the fine ore bins when she received a strain to her lower back. Employee received restricted duty.

Employee's left thumb was smashed between the forks and a threading machine.

Mechanic placed wheel chocks on the 553 haul truck and then walked to the front of the haul truck and felt back pain.

The employee was stripping airlines on the north MFL pads #9 and stepped into a low spot at which time he slipped and fell resulting in injury.

Contractor employee was removing a scaffold form bracket from a rack of material when the handrail bracket opened and caught the employees right index finger in a pinch point causing a laceration to the finger. Employee received restricted duty.

Employees were passing duct work by hand between one and another. The employee on the low end receiving the duct felt a pain in his right forearm. Employee was taken to medical facility for evaluation and was released without restriction or prescription. On the follow up doctors visit on 1/9/2012, the physicians assistant placed restrictions on the employee.

The injured employee was assisting in the movement of a rolling scaffold when contact was made with a light fixture. The light fixture fell striking the injured employee in the mouth requiring sutures to close the injury.

A Heavy Duty Truck shop mechanic received a laceration on his left cheek, when he struck a zipper link on a dozer track with a 4 pound sledge hammer and was struck in the face with a piece of fly metal. He received 8 stitches.

Employee got moly in his eyes, while helping to unsand a line. Employee was on his 3 scheduled days off and returned back to work on his next scheduled shift.

Employee's hand was smashed in a haul truck door after a gust of wind blew it shut.

Contractor employee was using a pry bar when the item he was prying on became dislodged causing his left 5th finger to become caught between the pry bar and a lifting eye attached to a motor nearby. The contact between the two objects casued a fracture to the tip of the finger.

At 11 AM on 7/18/12, EE was operating the controls with his right hand & leaning on flat plate that separated the lever control area from the drum that rotating drum with his fingers in the service port as the drum was turning. The bolt that holds the cable on the drum pinched his fingers between cable bolts & wall, resulting in a laceration to the top of his ring finger.

Employee was cutting a piece of electrical tape with his pocket knife, when it cut through the tape and cut his left index finger, requiring sutures.

Employee was grinding casing with a 9"" grinder and was rotating casing when he grazed the grinding wheel with his right thigh causing a laceration which required 12 staples.

Drill deck panels were removed to facilitate re-entry set-up. During this time the driller assistant returned from an errand and accessed the drill deck and fell through the open panels resulting in a laceration of the leg.

Employee was removing boulder that had landed on top of barricading sign. He and another employee were lifting the rock to allow another employee to pull sign from underneath. As sign was being pulled out, a smaller rock that was wedged between the sign and large rock, rolled across employees fingertips and pulled 2 fingernails back.

A mechanic was trying to knock out a bearing sleeve with a punch when the punch hit him in the mouth, chipping a tooth and loosening another.

Employee smashed his right ring finger between a rock and the chute wall. This caused a laceration.

Unclassified, insufficient data

Employee was in training on date and we had just finished respirator fit testing. We went on site to do a ground check on an AC unit. He watched another EE do the test and then left the mine. He was not working on the site at all. He did not notify National Commercial & Industrial Coatings of any injury. We found out through our workers comp/payroll service at a later date.
