An employee was removing a piece of pull point (LB) from conduit when the pipe wrench slipped causing the employee's right index finger to be smashed between the conduit and the wrench.

While a pump was being hoisted it contacted overhead grating, in attempt to guide the load, the employee pinched his finger between the crane block and cable.

Employees were attempting to install a 8""X8""X1"" steel diagonal brace. The upper end of the brace slipped off the connection clip on the support column and struck the right hand of Employee #1 pinching it between the diagonal brace and the inside of the support column. Physicians at UMC-Tucson made the decision to amputate the right pinky finger.

The operator of the 584HT was descending the stairs when she slipped and fell causing an injury to her knee.

An employee operating a tracked skid steer attempted to make a turn and contacted his co-worker pushing him between the track and an I-Beam of a wet screen stand.

Climbing back into the truc, he shifted his weight and felt his leg pull

Limited access to task location. While making adjustments to equipment employee stuck right elbow on steel crusher tire. Employee sustained a small puncture wound (.5mm) on elbow.

The haul truck operator stated he felt pain in his back on 7/24/2014 after running over a pot hole. Reported injury on 7/25/2014.

E/E was sprayed with sulfuric acid while working on the rail car unloading dock.

Employee was replacing the cam gates when he hit his head on an overhead beam. The employee was wearing his hard hat at the time.

Employee was exiting portable toilet, tripped, and fell to the ground. The employee was evaluated at the local health facility and found to have a fracture to her right ankle.

Employee was putting away a full size compressed gas cylinder when it fell and struck his left foot.

Employee stated on 4/22/2014 he felt pain in lower back while performing maintenance. Reported on 4/25/2014, evaluated by a physician, and was released from care. On 4/29/2014 the employee was re-evaluated by a physician and restricted from work.

Mechanic was stepping into the transmission compartment when his foot slipped causing him to twist his knee.

A SWI EMPLOYEE WAS PERFORMING HIS ROUTINE DAILY JOB(FLUSHING AND MAINTENANCE ON MFL PADS/LEACHING). HE HAS PERFORMED THE TASK FOR THE LAST YEAR AND A HALF. THE EMPLOYEE HAD FINISHED OPENING A FLUSH VALVE AND WAS WALKING TO WHERE THE OTHER 2 EMPLOYEES WERE AT THE OTHER VALVE. HE STEPPED ON A SOFT AREA AND HIS LEFT KNEE BENT INWARD AND IT CAUSED HIM TO FALL, LANDING ON HIS SIDE.

Employee was navigating a congested area when he caught his foot in a loop of cable, and spun around to prevent from falling backwards and twisted around while his foot remained stuck causing a fracture to his foot bone

Employee was removing a shim when a latch bar dropped contacting his finger resulting in an avulsion to left 3rd finger.

As the employee was exiting the cab of the FB005A Flatbed and stepped backwards instead of down causing him to fall landing on his elbow causing a fracture.

Employee was traveling down the haul road when he ran over spillage and felt pain in his back.

While stripping lines on a stockpile, an employee twisted their ankle.

Employee slipped while accessing the engine compartment causing pain in shoulder. On 8/1/20104 he was seen by a specialist giving him restrictions for work.

Employee was pre-occupied on a cell phone was not paying attention. The area that the worker was in was not a commonly traveled area. This area contained open holes that were not properly barricaded and protected. The worker stepped onto a piece of wood that was covering a hole that broke and worker fell with his left leg into the hole.

An electrical general foreman sustained flash burns after exposure to an arc flash. The employee was attempting to test continuity of 480-volt feeders going to a crane disconnect.

Employees were moving rolling scaffold tower on a slope when wheel turned and caught employee's head in between concrete pillar and scaffold.

Employee was stringing out welding leads and placed the welding leads over her left shoulder. Employee began to walk with the leads and lost her footing causing a fall to grade. As the result of the fall the employee's left shoulder sustained a small chip from cartilage from left shoulder socket.

Employee conducted a walk around inspection of his vehicle. As he was entering in the vehicle he felt and heard a pop in his left knee.

EE was on the second rung of a A-frame step ladder, step down lost balance and fell to the floor sustaining a fracture to her right wrist.

Employee was tightening a bolt and sustained a laceration to his finger

Employee was in the improper position during task and proper communications not used.

The operator of the 606 Haul Truck felt pain in her back after being rough loaded at the 12 Shovel.

Employee was operating the 631 Haul Truck when he ran over a pot hole causing pain in his lower back.

Employee was assisting with the removal of an inside duel tire on a F-350 flatbed truck, he had his fingers in a relief hole of the rim, when the rim dropped off the lugs it pinched his finger between the rim and hub causing a fracture to the tip of his right middle finger and a laceration that required stitches.

The employee was exiting a piece of equipment when he slipped and fell.

The operator of the SX-526 Broderson was retracting the outriggers when it rolled onto an employee's foot.

An employee was attaching a chain hook to a D-Ring and pinched his right pinky finger between the hook and the D-Ring.

The operator of the 518 Haul Truck was traveling up a road when he drove over a minor dip in the road and felt pain in his shoulder.

Employee was stepping down from the bottom step onto the ground when he rolled his left ankle.

An employee was returning from a break, as he entered the cab an shut the door he caught his middle finger in the door.

The operator of the 515 Haul Truck ran over a rough patch in the road and felt pain in lower back.

E/E was loading 55 gal barrels into the bucket of a skidsteer. During the process the E/E pinched a finger on the left hand between two barrels. E/E was treated at the local clinic and given duty restrictions,

Employee rolled ankle when he was exiting back of service truck, The ankle was swollen enough that the employee could not put his boot back on.

Employee was helping install a grating panel across a launder wall, when the tip of his left pinky finger was pinched between the grating being placed and an existing panel already in place.

An employee moved in a twisting motion in his seat to obtain a better view and felt pain in his back.

On 6/19/2014 the employee slipped and fell on the 2nd step of the Haul Truck causing pain in her shoulder, she was evaluated and released without restrictions. On 6/21/2014 the employee returned to the clinic and was given restrictions. On 6/26/2014, employee was released to full duty but returned to clinic on 7/1/2014 and was given restrictions.

Employee twisted her ankle while stepping out of a van. Employee returned to the clinic on 6/16/2014 with continued pain and was given work restrictions.

Employee was attempting to hang a lifting cable when the cable fell, applying pressure to his left arm. Employee reported the injury on 6/10/2014, at 01:00am.

While attempting to stand up the employee's foot slipped causing his foot to become caught between two cross beams.

Employee was walking to the 10R Drill employee stepped into a low spot causing pain in his leg.

Employee attempted to move a beam by hand and as he shifted the beam his left glove got caught between the beam and cribbage it was resting on, resulting is smashing the flesh of his left hand middle finger tip.

Employee stepped over scaffold ladder, tripped, and fell at same level. Caught weight with L hand and sustained injury. Returned to Safety on 04/22/2014 with unresolved pain. Referred for X-Ray.

EE stepped out of a pickup truck bed and lost balance and sprained ankle. EE was given duty restrictions.

Contract employee injured his right shoulder when he stepped over a concrete curb, tripped, and fell at the same level. On 04/02/2014, a physician diagnosed a suspected tear of shoulder connective tissue and restricted the employee's duty.

Employee was grinding a metal pipe to weld, was wearing all necessary PPE, went to remove her PPE and the metal dust entered her right eye. Eye wash station was accessible and used. After washing the eye she was sent to see a specialist to ensure everything was ok.

Employee was securing bolts using a ratchet when it slipped contacting the employee on the back of the right hand.

While changing the #4 tire the inner ring slipped contacting the employee's finger causing a fracture to the tip of his finger.

Employees were removing a take up pulley with comealongs. One of the rigging points had a slight side load on it causing the pulley to drift when the final bolt was removed. The pulley contacted the employee's left thumb in a pinch point on a channel iron causing a compound commuted avulsion.

Employee was working on the left side of a scraper kneeling down next to a mechanic truck. The operator of the truck honked twice and pulled forward not knowing that the other mechanic had stuck his foot in line with the rear tire. The edge of the rear tire ran over employee's right foot.

Employee states he felt pain in his back.

Employee was walking when his knee buckled causing him to fall. The employee reached out with his right arm to catch himself and felt pain in his shoulder.

Employee stepped out of material trailer and caught his foot on debris on the deck. He tripped and fell approximately 1 1/2 feet to the ground. He landed on his left wrist which resulted in a fracture to the scaphoid bone, between the thumb and wrist

Employee was rough loaded and experienced back pain.

Injured caught his thumb in a rigging chain while setting a power pole, received lacerations only with stiches. Return to duty same day.

Setting pipe the employee grabbed the pipe to control the swing and caught his finger between the pipe and Cable tray causing a laceration

An employee was walking when he tripped on a rock and fell causing a laceration to his upper and lower lip.

An employee sustained a laceration to his left thumb, while cutting duct tape with his pruning sheers. The employee was attempting to remove an unusable portion of tape from the roll. Medical treatment consisted of five sutures to his left thumb below his second joint. Employee was released back to work with no restrictions. Follow up in 10 days for suture removal.

An employee was picking up trip cable that had been banded into a loop. As he tossed the bundle over his tailgate into the bed of the truck, the metal band contacted his left hand causing a laceration to the palm.

An employee was operating the lathe when the scale snagged on the bushing causing a laceration to his finger.

Employee was tightening a bolt when the socket slipped off of the bolt and the torque wrench with an extension came up and struck the individual in the mouth chipping the two front teeth.

An employee was removing track pins at the 5400 lay down yard, while using a 12 pound hammer, with duct tape on the head, to break loose the pin, a piece of fly metal hit his right cheek.

Employee was using his personal pocket knife to pry a bent piece of metal when the knife slipped and punctured his left wrist.

Employee was operating a 637E scraper and lost control when his front tire went over a windrow causing the scraper to bounce. The employees hardhat fell off and he went forward hitting the window latch causing a cut on the top of his head, requiring one staple.

Using a come along, he repositioned his hand to start pulling pipe, the tension of the twist in the chain unwound, and struck him in his mouth chipping his tooth.

While attempting to remove a ball from the liner the eye broke off and struck the employee in the cheek causing a minor laceration to the right cheek.

Employee was attempting to tighten an air line when he struck above the eyebrow with a wrench causing a small laceration.

Employee was removing pre-strip copper when the plate swung towards him and contacted his arm causing a laceration.

On 7/15/2014 an employee was attempting to hook up a trailer when he felt pain in lower back, but did not report the injury until 7/17/2014.

Employee struck his right hand on the bottom of a hub liner causing a small laceration on top of his right hand. Employee was wearing leather gloves at the time.

The employee was removing his welding hood when a foreign body entered his left eye. The employee was treated for a foreign body embedded in the eye. The employee was treated and returned to work without restrictions.

Employee was moving c-channel steel with coworkers. The steel shifted and struck the employee's boot, which was steel toe. The employee was treated and released to work, but returned on the morning of 6/24/2014 with pain and swelling to right 3rd toe.

EE was leaning on a delineation cable at East end of New Moly Bld, the cable was secured with cable clamps. The cable came loose from clamp appt 20ft from the employee. The employee fell apprx 3.5ft to the ground striking his head causing a laceration

Employee was tightening a bolt when the wrench slipped causing him to strike his finger against the support beam resulting in a fracture to the pinky finger on the left hand.

Employee shut his finger in the door of the pickup causing a small laceration to the middle finger on the right hand.

While employee was carrying a conduit, he slipped and dropped the conduit. As he was catching himself he jammed his right middle finger on the conduit.

Matrix employee was fitting pipe on the second floor of the Ball Mill South East corner of cyclone #1. Scaffold Builders started to work where he was walking through so he went around the south east corner and tripped with the Mule Tape that another contractor left, landing on his right hand under his chest

Employee was struck in the abdomen by fly metal while using a sledge hammer to remove ball studs.

Working with 2 fellow employees on the West Side of the Wet Screen using a Hydraulic torque wrench, one employee positioned the torque wrench over the nut, the second held the bolt in w/a hammer wrench braced against the structure & a third activated the hydraulics. EE pinched his finger between the structure and the hammer wrench

Employee was cutting 24"" HDPE when the chainsaw kicked back causing a laceration to the lower leg.

An employee was replacing cutting edges on the VT-1 Loader when his tool slipped and he received a laceration to the hand.

Employee was using a spud wrench to loosen bolts when the tool slipped, struck him to his mouth, and chipped two front teeth.

1:50am a Matrix employee was working at thickener #3 tightening a 3/8 90 degree tubing fitting. The 8"" crescent wrench slipped off of the tubing fitting causing his right hand ring finger to come in contact with the sharp metal corner of the name plate and pump mount bracket below, causing a 1"" laceration to his right hand ring fingers 2nd knuckle.

Two employees hand loading a 45 pound poly barrel on a flat bed truck and one employee lost grip of the barrel and his hand slipped under the barrel catching his finger between the truck bed stake hole and the barrel cutting the tip of his finger. Equipment (fork lift and pallet) was available to load the barrel onto the truck procedure not followed.

Employee was walking to work area with welding rods in his hand and when his welding hood was falling down over his face he attempted to catch it with the hand that he was carrying the welding rods in and poked his eye under his safety glasses with a welding rod.

Cutting the weld on a piece of unistrut with a 6"" metabo the disc bounced off the unistrut off the I-beam hitting the finger on the left hand causing a laceration . The employee did not have a proper grip in the metabo and when it bounced off the metal his hand came free and the disc cut through his glove cutting the top of his finger on his left hand requiring sutures.

The employee was walking across the Thickener #4 floor stringing out welding lead and slipped on a piece of gravel. She was evaluated at the local urgent care facility and diagnosed with a strained back.

Millwright was moving a 2'7"" x 2'7"" x 8'2"" sampler duct. His left index finger became trapped under the duct causing a de-gloving at the 1st knuckle of his left index finger.

Employee received a laceration to the leg while trying to manipulate a metal plate.

Groundman was releasing tension off chain hoist after installing a down guy and lost control of the handle. As a result, the handle ratcheted back toward the employee striking and fracturing the left thumb. Employee received surgery to re-set and pin the injured thumb, and a soft cast was issued.

EE loss control of angle grinder struck right knee sustaining a laceration that required sutures to close, returned to work.

EE was manual handling form brace and struck an object that pinched his finger resulting in a lacatration that required sutures to close the lacatration, EE returned to work.

While removing his welding hood, the employee felt a foreign body enter his left eye.

EE received a laceration to the back of his left hand as he reached out to lift up on a rebar mat which was about to contact a gate post and contacted the sharp edge of the rebar. Sutures were required to close the wound.

Employee was grinding on a dipper when he felt something in his left eye.
