While a pump was being hoisted it contacted overhead grating, in attempt to guide the load, the employee pinched his finger between the crane block and cable.
Employees were attempting to install a 8""X8""X1"" steel diagonal brace. The upper end of the brace slipped off the connection clip on the support column and struck the right hand of Employee #1 pinching it between the diagonal brace and the inside of the support column. Physicians at UMC-Tucson made the decision to amputate the right pinky finger.
The operator of the 584HT was descending the stairs when she slipped and fell causing an injury to her knee.

Contract employee injured his right shoulder when he stepped over a concrete curb, tripped, and fell at the same level. On 04/02/2014, a physician diagnosed a suspected tear of shoulder connective tissue and restricted the employee's duty.
Employee was grinding a metal pipe to weld, was wearing all necessary PPE, went to remove her PPE and the metal dust entered her right eye. Eye wash station was accessible and used. After washing the eye she was sent to see a specialist to ensure everything was ok.

Two employees hand loading a 45 pound poly barrel on a flat bed truck and one employee lost grip of the barrel and his hand slipped under the barrel catching his finger between the truck bed stake hole and the barrel cutting the tip of his finger. Equipment (fork lift and pallet) was available to load the barrel onto the truck procedure not followed.

Millwright was moving a 2'7"" x 2'7"" x 8'2"" sampler duct. His left index finger became trapped under the duct causing a de-gloving at the 1st knuckle of his left index finger.
