Employee was installing impeller and was spinning the belt to tighten impeller on to shaft. The belt was moving faster than the employee expected and left hand index finger was caught in-between shive and the belt.

A mechanic was changing the ripper shank when the guard shifted and smashed finger between the guard and the shank. ***Per phone call distal tip of left index finger, a very small amount of bone was amputated.***
An employee was walking down the stairs and twisted their knee.

When EE was approximately 500 yards inside the tunnel from the tail end side, the right rear tire blew. The employee stated that something, EE was not sure what, knocked off EE's hard hat and respirator.
An employee bent over to set lunch box down when stood up EE felt pain in back.
An employee was walking down the steps carrying a 50lb bag of clay when ee missed the bottom step and fell, landing on right knee.

While working on the tracks at the South West Mine for Leach Radial Stacker #3, an employee was standing back at approximately 111 feet away from the track. While watching the track make a move ee felt something hit face. EE walk over to a co-worker and ee was bleeding. EE was taken to the clinic and a piece of fly metal was found lodged in upper lip.
Upon entering transport van 0068 employee felt a pain as if something had bitten EE. Once on the job site EE noticed a swollen ball at the top of EE's hand. The employee was transported to clinic and diagnosed as a bug bite.
An employee was skinning insulation when knife slipped causing laceration to finger.
An employee was fueling a welder. On the third attempt, the fuel came out and a small fire started catching the mechanics shirt on fire employee then jumped to the ground rolling around attempting to put fire out.
EE was positioning liners when the mechanic on the outside of the chute was assisting with a spud wrench to line up the liner to insert a bolt. The ee in the chute got finger in the bolt hole of the liner, while the employee out of the chute inserted the spud wrench to line it up. The finger was smashed in-between the liner and the spud wrench.
While walking from the parking lot to the office, an employee tripped and fell down. The force of the fall caused a rib to break and left employee with minor scrapes on hands.
Employee was climbing stairs and as EE turned right EE felt a pop in right knee.
An employee was walking down the shovel access stairs when a co-worker mistakenly activated the ""up"" mechanism causing the employee to stumble and experience pain in left knee.

The door hit the employee on the right side of the head giving ee a concussion. Employee was wearing hard hat and all appropriate PPE.
On 12/8/2016, an employee was at the tricanter to do clean up of electrolyte and organic. As employee stepped off the curbing and onto the yellow ramp, they slipped, landing on their side hurting their right ankle. On 12/9/2016, employee was given restrictions.
On 12/5/2016 an employee was shoveling material for approximately 1.5 hours and felt pain in left shoulder. On 12/10/2016,employee was seen in the clinic for continued pain and was given restrictions.
Employees were unloading a 4x4 20' long square tubing from the flat bed. When the employee on one end of the tubing, lifted it off the cribbing. It slipped out of employee's hand, and smashed it between the cribbing and the metal. The metal cut through the employee's leather gloves and employee's fingers.
An employee was driving when EE ran over a pot hole causing EE to bounce in EE's seat and strike EE's chest against the steering wheel.

Employee was assisting a contractor in a 54"" Conveyor belt replacement on the RP1 Radial Portable Conveyor when right middle finger was injured. The group was preparing to lift three 54"" troughing idlers with the 75 ton Linkbelt crane when the employees finger was caught between the grab chain and an idler frame.
An employee was harvesting, noticed a low hanging edge strip so flew the load to the bottom of the back bay. Then used a hand tool to lift one side of the blank off when felt a sharp pain on the right side of back.
An employee had squatted down to turn a valve when ee feet slipped out from under causing pain in ee groin.
Employees were tasked to change out the bad idlers on the p-11 and the p-5 belts. They were on the last idler for the p-11 belt and were going to stage the lifting bar with the come-a-long. While EE was carrying the bar and the come-a-long EE stepped on the soft mud and the concrete causing EE to slip and fall backwards onto the ground landing on EE's left side.
Employee had rested a 57"" lock ring against a tire. EE was attempting to lift the bottom half up so EE could slide it onto the top of the tire. The lock ring weighs approximately 103 lbs. As ee was lifting upward ee felt a pop in right shoulder. This task is completed on a daily basis and is routine in nature.
On 7-22-16 the employee was shoveling in the #6 ball mill 7-conveyor pit and experienced pain in EE's back. The employee reported it to supervisor and went home after the shift. The employee returned to work on 7/23/2016 and reported that EE has pain in EE's back.
On 7/16/2016 between the hours of 8:00am and 11:00am, an employee claims to have driven over a rough patch in the road. The employee reported the incident on 7/20/2016 and taken to the clinic for evaluation & was given restrictions.
On 6/17/2016, an employee was pulling a wagon of materials along the center line dam from behind, when EE was maneuvering around a valve and the wagon got caught and jerked employee causing EE's left shoulder to pop. On 6/20/2016, the employee was re-evaluated at the clinic and given restrictions.
On 6/15/2016, an employee was walking when they felt pain in their knee. They did not report the injury. On 6/16/2016, they reported the pain and requested to be evaluated by a medical professional. On 6/20/2016, the employee reported continual pain and was given restrictions.
On 6/11/2016, an employee was walking under the cell line when EE slipped and began to fall. EE grabbed onto a pole and caught self before EE hit the ground. On 6/12/2016, the employee returned to the clinic with continual pain and was given work restrictions.
On 6/11/2016, an employee was cleaning and came in contact with the slurry. The slurry caused the employee to feel a burning sensation. On 6/14/2016, the employee was given minor restrictions, however, was on days off until 6/19/2016, and was released to full duty on 6/20/2016.
Employee was conducting maintenance when EE was struck on the foot by the edge of a liner causing a severe bruise to the foot. The employee was taken to the clinic. The following day, 6/1/2016, the employee returned to clinic and was given restrictions.
Employee was torqueing the right final drive mounting bolts on the 616 haul truck with an air regulated torque wrench. The socket slipped off of the bolt and slammed Employee's hand against the final drive.
Two employees were installing a bearing. When one employee swung the hammer attempting to strike the bearing ee inadvertently struck the other employee in the hand with the hammer causing a fracture to the hand.
On 3/31/2016, an employee tripped and fell causing ee to land on shoulder. The employee did not report the injury until 4/1/2016 and was seen by medical staff. On 4/2/2016, the employee attended a follow-up appointment and was given restrictions.
On 3/7/2016, an employee was removing the thermal coupling when ee noticed a slight pain in wrist. The employee reported the pain on 3/8/2016 and was taken for medical evaluation. On 3/9/2016, the employee was given restrictions by the physician.
Employee was walking across service bay. While walking, EE's ankle buckled. EE heard a popping noise and EE was unable to stand with full weight on EE's foot.
An employee was driving a pin from a stiff arm when the pin released and the stiff arm dropped slightly and pinched the employee's hand causing a laceration to outer palm.

Employee had positioned self laying across a belly pan to work on the coolant hose. EE finished the job and noticed discomfort and pain in left lower back.

On 8/16/2016, an employee hit EE's shin on the bottom ladder of the truck as EE was getting aboard the equipment resulting in a minor cut. On 8/29/2016 the employee was treated for a secondary infection.
Employee was pulling a blank out of the bottom of the machine when it fell on finger causing a laceration. Employee was wearing leather gloves.
While cutting slope drip, utilizing the cutters in right hand, the employee lost footing and as was falling, reached out with right arm to catch self from the fall and in so doing cut the inside of left arm approximately six inches above the wrist.
On 5/25/2016, an employee was conducting a pre-shift inspection when felt something in right eye. EE rinsed eye with water but continued to feel irritation in eye. EE began to rub eye and then rinsed again with water. The employee was on scheduled days off from 5/26/2016 - 6/2/2016.
Employee was changing out skirt board liners, EE went to push the liner out of the way and misjudged the distance smashing EE's right index finger. The skirt board weighs approx. 30 pounds.
An employee was shoveling when the handle broke and employee received a laceration to the right finger.

On 4/1/2016, an employee working assisting with a shovel move. As ee applied tension to the rope to remove the slack. The rope broke causing the employee to stumble backwards and fall striking elbow on the base of the cable tower. On 4/8/2016, the employee was seen in Urgent Care and treated for an post injury infection.
An employee tossed a chain over a driveline when it swing back and struck in the mouth causing ee tooth to chip.
Drill crew were advancing 16 x 10 sections of casing. The driller did not realize that the 16 casing had moved up with the pipe into the bottom of the drill table. A blast of air and cuttings blew out one of the bushings that hit the driller on the back of EE's hard hat. The employee received 7 stitches and was released to full duty.
An employee was cleaning drill parts when EE felt something in eye. EE flushed eye but continued to feel irritation. The employee worked the remainder of shift and the following morning reported to the supervisor that left eye was irritated. Employee was taken to the clinic and treated for a minor infection. No foreign bodies found in the eye.
An employee tripped while walking across the deck of the haul truck and struck the fire extinguisher resulting in a laceration to the chin.
