EE was preparing to install a final drive on a D8 dozer. EE began preparing the surface of the main frame for the install of the final drive. The forklift rolled forward causing the final drive to flip forward off the forks striking EE in the leg pinning the leg between the final drive and roller frame.

While working to gather materials for the job, the employee bent over to pick up an item weighing only a few ounces. While standing back up, the employee felt a sharp pain in employee's back. The pain persisted after the incident had occurred and the employee decided to seek medical attention.

Employee was grabbing the stinger to hang it up and the pinky finger of left hand got ""pinched"" between the stinger and the guard rail. Employee was attempting to lift the stinger over the hand rail to place back into holder.
IP was handling the wireline with hydraulic controls when the overshot became hung up on the bottom of the deck causing an uncontrolled release of energy resulting in overshot to contact the IP in the lower lip. This resulted in a laceration between the lower lip and chin and chipping one lower tooth.

A contract carpenter was cleaning out a tailings launderer in the tunnel on the south side of Metcalf Mill in preparation for pouring concrete to repair the launderer. There was a piece of broken rebar under the standing water and it poked through the side of the employee's boot and foot.

Employee has working on HT567. After bay was prepped, EE was walking in the bay to get a tool for another employee. While walking EE felt a pop in left foot. Employee was Diagnosed with plantar strain/plantar fasciitis

EE sustained a boxer fracture to the right hand when EE stepped off the 1-foot drill deck and slipped on ice and frozen ground. EE attempted to throw drill rod away from EE, but it bounced back and landed on EE's hand. EE was wearing impact resistant gloves at the time of the injury as part of the required PPE.
Employees were working on a upper feed box installing liners and protectors when the jack stand got hit and shifted the feed box employee on the back side of the feedbox got hit/smashed by it. Employee was removed right away and taken to hospital.

Driller in training was pulling slack out of mainline when overshot caught up on the choke/ weight ball and voke, overshot swung over and struck driller on the mouth resulting in a cut lip and chipped tooth.
While tripping in drill pipe, drill tech was jiggling the holding fork free, when EE's right ring finger made contact with drill table. EE was wearing impact gloves, sustained a small fracture, cut wound to tip of the finger. Finger was stitched, splinted, and given prescriptions. Released to work normal duty, to tolerance.
Employee was working on a chute, a gust of wind came through the area blowing dust under the employees Safety Glasses. The employee felt discomfort, left the area and reported the incident to Supervision.

Employee was in the process of picking up a piece of channel iron that was cribbed on wood blocks. While picking this c-channel up another piece of c-channel moved-tipped into EE's right thumb causing an injury. The injured employee was wearing EE's glove and proper PPE.
