Our mixer driver was hauling for Freeport McMoRan at their Morenci mine location, the load was rejected and sent back, driver followed the escort down a hill with a full load, lost brakes and turned the mixer on its driver's side.

Employee states they were descending the haul truck stairs when their ankle popped.
Employee was walking to turn off water supply when they twisted their knee and ankle.
Following loading, the truck driver could hear something and thought maybe something wasn't tight on the top of the tank trailer, and it may leak. EE was checking cap on top of loaded trailer for a bad gasket. They started to loosen the ear on the cap, and the cap blew off with sulphur pouring out behind it, spraying EE with molten sulphur.
Employee states their knee started hurting after walking through out the shift.
Employee was walking across a mine stockpile when they stated their knee popped.
Haul truck operator was rough loaded when large rock was placed in bed of truck.
An employee was attempting to open the door using a crow bar and was struggling. Injured employee moved over to the location and using the same bar assisted them in pulling on the bar to get the door open. The bar either slipped out of position or they broke the door loose causing them to loose grip on the bar and striking injured employee on the wrist causing the fracture.
Employee states they had a brief exposure to the dust from a fire suppression system that accidentally activated on their drill.

A Haul truck driver had just completed their pre-operational inspection. After lifting the access stairs, the operator turned hitting their knee on hinged spring for the gate.
Employee was dismounting the crane when they slipped causing a strain on their arm and shoulder.

Employee was pulling large gauge cable when the cable came loose from the tray and the employee put their right hand on the cable to steady it, resulting in a fracture of the #4 finger of the right hand.

Employee was walking back on the road from the ew shop after dropping off forklift to the 6 yard to take a picture of delivery. EE tripped on a rock on the road and landed with EE's right knee on a rock. EE was able to stand up then walked about 10 ft and fell back down on the road.
An employee was walking on the north side of the shop, stepped off a 3"" step and felt a pop in their knee. The employee was not carrying anything at the time.
The scaffold builders were getting ready to pin runner into the coupler and the affected scaffold builder had accidentally pulled down on the scaffold pipe down towards self instead of downward when grabbing the brace. The other employee still had a hold of the brace when it hit the affected employee. Affected employee was struck on the hardhat and then on nose.
A wooden core used to hold conveyor belt on the belt winder was bumped and rolled down the discharge conveyor and struck one of the mechanics in the back of the head and shoulder.
Was on top of conveyor system loading lime from railcar to tanker trailer when the wind picked up heavily blowing lime dust falling from conveyor into the air. Employee was wearing spoggle, clear safety glasses, bur felt dust enter the spoggles. At the time they did have a reaction to the dust and kept loading.
EE pulled on the RW scale to scale the truck in. As EE was climbing out of the truck cab, the top step (attached to the battery box cover) gave way, and they slid to the ground with it, landing upright on their feet, on left leg,(potentially with knee locked straight). Following, EE was unable to put weight on it. x-rays showed knee broken at the top of tibia.
Employee was spraying lower level of Tower, fellow employee started spraying level above. Overflow splash from top level hit employee in the eye.
This trainee just received permit and was riding along with our mixer driver for training purposes at the Freeport McMoRan Morenci mine location. The cement load was rejected at the job site and sent back to their batch plant, driver followed an escort down a hill, lost brakes and turned the mixer onto the driver's side.
Employee stepped out of their vehicle into a muddy hole. This resulted in the employee falling over and breaking their left leg.
Employee was stepping off a drill ladder and twisted their knee causing a sprain.

The employee was tasked to change out the #2 regrind cyclone pump. Employee removed guards, loosened and removed belts, to perform this task the pulley on drive motor has to be removed. While the contract employee was removing the pulley taper lock bushing, EE was working it back and forth to remove, when it came off, EE pinched EE's finger between the pully and shaft.
Mechanic was descending stairs on mobile equipment when their foot slipped Employee didn't fall but had pain in their lower back.
Mechanic was conducting field repairs of mobile equipment when they rolled their ankle after stepping into rut.
EE1 was prepping to install a 793 driveline spider onto the transmission. They cautioned EE2 below to step aside while they completed the task. When EE1 grabbed the spider to install, it slipped out of their hands and fell below. EE2 was struck on the left foot behind the steel toe.

While air lancing, the lance made contact with the below-ground impeller and pushed the air lance. In the air lance being thrust, the employee's hand contacted the sump pump structure.
On 4/14/22, An electrician was lifting a small valve mount onto a shelf in a box. The employee stretched to get the extra distance, causing their leg and abdomen muscles to pull. On 4/19/22, after a follow-up, the employee received restrictions.
The Haul truck driver was leaving the dump and hit potholes past the crest of the hill. The operator proceeded to the tiedown and reported they felt pain in their back.
On 4/13/2022, a mechanic carried a set of wrenches when they felt a pop in their left foot followed by a burning sensation. On 4/14/2022, after a follow-up, the employee received restrictions.
On 4/13/2022, an employee was getting in a vehicle; when the employee used the inside handle to get up, the employee reported they felt discomfort in R forearm. On 4/14/2022, after a follow-up, the employee received restrictions.
The employee's right middle finger got caught in between steel blanks when they adjusted the blanks on the rack. The employee was wearing appropriate PPE, but the pinch was enough to fracture the tip of their finger.
On 4/6/2022 while descending the stairs from the Moly press the operator tripped before the landing and bent their wrist on the hand rail catching them self from falling. On 4/7/2022, during follow-up appointment employee received restrictions.
Employee was moving a cathode, as they turned to start walking down a slight slope they slipped, tried to regain balance and let go of the cathode. The Cathode landed on the employees Right foot.
A haul truck operator reported tightness in their back after a rough load event.
Drip crew were pulling Mantos and felt the wind kick up. They knew they had to move out of the way because it would start flying up. Injured employee was walking out of the way when the Mantos pulled the 4"" pipe, the pipe then swept their feet from under them causing them to fall onto their back.
At the Fuel Dock, employee sustained a fall from same level due to ankle giving out. The fall resulted in a fractured elbow and wrist.
An Operator was going into discharge chute to access the dry screen lower deck. As they attempted to climb onto the first bench of the chute they slipped. Employee states their legs and shoulder contacted bench being accessed.
On 3/13/2022, employee reported that while mentoring trainee, sustained some discomfort in back due to the chair. Progressively the discomfort increased and reported higher levels of discomfort on 03/19. Employee states this is due to road and adjustments on the mentor chair for haul truck.
An employee was walking down some stairs when their right knee gave out causing them to fall and hit their knee.
Employee was taking samples at the cleaners regrind, when they felt pain and lost grip strength in the left lower arm.
On 2/28/2022, a handrail reclined on the wall fell over, hitting the employee in the Left Shoulder (The employee declined medical evaluation). On 3/4/2022, the employee requested to be seen at the local clinic.
An employee was walking down the blast pattern when their knee went out and twisted.
Employee was reaching in their pocket for gloves and EE's knife was out of the casing and cut EE's left thumb.
Employee was picking up heavy rocks to place onto the plastic covers on the leach field, when they felt pain in the left lower abdomen.
The Haul truck operator experienced slight discomfort on their left shoulder after a passing haul truck clipped the right batwing on their haul truck. The impact jolted both haul trucks.
An operator was walking out of the control room to check the B-Train, felt their knee buckled. The employee was taken to the clinic and given restrictions on their second follow-up on 2/12/2022.
A small crew was repairing a 6"" Carbon Pipe in the Moerenci Mill. With a crew member at each end of the pipe one member lost grip due to the wet conditions allowing the pipe to fall about 10"" striking the top of the thumb on the right hand. Causing a small laceration on the thumb.
A Concentrator employee was in the process of ball charging media in the regrind. The employee at opening the regrind's door felt discomfort in lower back.
An employee reported pain on their hip as they were exiting the man van.
A mechanic had their right hand on a new 64"" idler and left hand on the belt structure underneath the belt. As they were guiding the idler into place, it slipped off their shoulder and right hand and fell 2-3"" landing on their left hand. This resulted in lacerations to the skin on their left ring finger.
Employee at the leaching pad was whipping drip lines when they felt pain in their left shoulder and rip cage area.
An employee was working under the B Cell line when they bent over to walk under a crossbar. As they stood up, they ran into it causing their hardhat, face shield and glasses to smash into their nose causing a laceration.
Employee fel1 after stepping in a sample hole causing a sprain on their right wrist and ankle.
An employee was walking back to the cab from the back of the truck and stepped into a pothole. The driver could not see the hole as there was standing water covering it.
EE2 was installing a liner pack into number 3 cylinder on the engine of 504 in bay 15. EE2 called down to EE1 for an additional tool. EE1 got the tool and reached up from below to hand it to EE2. EE2 turned to grab the tool from EE1, during that time EE2 bumped a 12-inch adjustable wrench that fell and hit EE1 above the left eyebrow.
Knife slipped while employee cutting conveyor belt lacerating finger.
Employee was dismounting haul truck when they caught their leg on an open AC compartment box causing a laceration
EE was injured removing the verticals supports from the Ventilator affixed to the roof at Stargo. The blade bound on the support causing it to fall and cut EE on Chin.
Employee was removing tooling from the rigid toolbox behind the cab. The lid came down on top of the drillers helmet forcing EE's head down into the toolbox. The mouth of the driller bumped the steel toolbox frame that resulted in a chipped tooth.
Employee stated dirt fell behind EE's glasses while working on electrical wiring under service truck.
Employee states they felt tightness in their back when they laughed at a co-worker while riding it light vehicle.
While hammering on a bin being repaired, a piece of debris fell into their eye.
Employee was walking down a concrete ramp when they detoured causing them to trip on another ramp.
Employee was at the pump house opening up the double doors when something stung their arm.
Employee was dumping a wheel barrel full of fines when it hit a beam causing dust to fall into their eyes. Employee was wearing safety glasses.
Inspecting geomembrane lining installed on silver Basin Cell 3. Slope was wet due to previous rain. While descending the slope IE's one foot started to slide. EE tried to walk the rest of the way to bottom of slope when EE tripped on a sandbag falling onto face and chest onto the liner.
As one of the employees was picking up material from the pile, the other employee had bent down and was going to pick up material with EE's shovel. While EE was leaning down, EE was picking up material and contacted another employee, causing a cut on EE's forehead.
Employee states a piece of metal got into their eye while working close to contractors grinding some metal.
An employee was cutting a 50 foot section of pipe into 4 foot sections to be discarded using a box cutter, in doing so the employee lacerated their left thumb requiring 1 stitch to seal the wound.
The Employee was trying to free an auger at the Lime Plant by removing the lime around the auger with their hands, exposing the skin to the lime they suffered 2nd degree burns on the left wrist.
Was a piece of metal propped on the wall sticking out slightly. EE's right hand make contact with the metal causing a small cut on EE's right hand. Employee did not notice the injury until a few minutes later. EE called the supervisor and Safety. Employee was taken to GHR. Employee will be a medical treatment.
A Tire Shop repairman was preparing to air up the #1 tire on a haul truck and connected the ""big air"" hose connection onto the valve stem. As they made the connection, they noticed the big air hose bleeding off, so they tightened it, and as they were doing it, air bled through with enough pressure to make the hose whip around and strike them on their chin.
A mechanic was loosening nuts underneath the cutting-edge dozer when the spline gun being used slipped off a nut and struck the individual in the face. The employee sustained a cut to the lip and a chipped tooth.
Employee was using a spike lifter to loosen rusted rail spikes. When the spike head broke and hit the employee on the left side of face (chin) causing a laceration.
Employee was assigned to clean under ball mill feeders shoveling material. The employee reported they noticed soreness to the left leg upon completing the task but did not remember any moment where they were injured. The employee continued with their scheduled days off. Upon returning on 3/1/2022, the employee reported the incident to their supervisor.
Haulage Employee was shadowing the Blast crew at phase 14. While the employee was walking at the logging pattern, the employee slipped on a rock. Employee hit their R Knee. Resulting in the employee's knee laceration required sutured.
At approx. 1300, a diagnostic electrician was cutting conduit for a receptacle line. While the portable band saw was coasting to a stop, the employee made contact with the blade along the top of their right hand. Employee was taken to the clinic and received 3 sutures.
Haul truck operator was draining air tanks at start of shift. The air created a plume of dust. Some of the dust got into their eye causing irritation.
01/11- Mechanic was descending the drill ladder when the last step broke on the welding causing the mechanic to fall landing on both feet. 01/19 - Mechanic was referred to PT.
Incorrect shelving used and was not regularly inspected for defects and security
Employee was cutting hose with a pocket knife. Knife slipped off the hose cutting employees left index finger requiring sutures.
Employee's asthma was aggravated by smoke from a small vehicle engine fire.
Employee was operatring ATV all morning on the leach stockpile when they started feeling nauseous, light headed and weak.
Employee has had ongoing wrist pain, diagnosed as carpal tunnel syndrome.
Employee reported left wrist was bothering them which was diagnosed Triscaphe osterothropathy with subcortical cyst at base of thumb.
Employee was operating a valve to turn a water hose on when they sprained their wrist. Update: Physician changed the diagnosis to tendonitis due to repetitive use.
Employee was found unresponsive and lying on the cab floor of the electric shovel EE was assigned to operate.
Prior to the beginning of EE's work shift, EE experienced what was reported to contractor as a pulmonary embolism in the mine parking lot which resulted in EE's death. It is contractor's understanding that the incident is classified as Degree 8-Natural Causes.
